Box 221 Helper, Utah 84526-0221 Subject:Plan Approval, Helper City Transmission Pipeline Replacement Project, Distribution System (DS001); Helper City, System #04004, File #15848, SRF #3F1880 Dear Lenise Peterman:The Division of Drinking Water (the Division) received the plans and specifications for the proposed Helper Transmission Line Replacement Project from your consultant, Lauren Ploeger, P.E. and Franson Civil Engineers on July 19, 2024. Review comments were provided to your consultant on August 13, 2024. The Division received the revised plans and specifications on August 15, 2024. This project has met the conditions for receiving a plan approval. Our understanding of the project is the replacement of approximately 19,200 feet of transmission line from Royal to Helper with 10-inch diameter HDPE pipe, the replacement of approximately 1,780 feet of 10-inch diameter HDPE transmission line from Colton Well #1 (WS003) to Helper City’s Spring Canyon transmission line. Also included is the installation of an interconnection between Helper City (04004) and Price Municipal Corporation (04007) culinary water lines at Royal. We have completed our review of the plans and specifications, stamped, and signed byLauren Ploeger, P.E., dated August 15, 2024, and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for Helper City Transmission Pipeline Replacement Project are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before the Helper City Transmission Pipeline Replacement Project may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information. Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.The project, as described herein, is generally eligible for assistance under the state Drinking Water State Revolving Fund Financial Assistance Program. All change orders must be reviewed and approved for compliance with the Drinking Water Rules, the conditions of this construction approval, and the DWSRF Program’s financial assistance requirements.The project may now be advertised for bids. But, as a condition of the project funding, you must not award the contract or proceed with construction until the Division of Drinking Water has authorized you to do so. This is necessary to comply with applicable requirements and to protect you, as the funding recipient, by ensuring that the contract meets requirements for funded projects.
