Dear Mr. Manning:
As requested, Rockwell Solutions, Inc. has completed a Phase II Environmental Site Assessment at
the above-referenced facility.
SITE DESCRIPTION
The site is an active gas station and convenience store located at 325 West Main Street in Hyrum,
Utah. The site has two 12,000-gallon and one 8,000-gallon above ground storage tank (ASTs) in an
earthen containment on the north end of the property. There are three dispensers under the canopy
east of the convenience store. A site map is provided as Figure 1.
METHODOLOGY
Six borings were advanced on July 10, 2024, to evaluate if soil and groundwater had been impacted
at the site. Three borings were advanced around the ASTs and a boring was advanced adjacent to
each of the three dispensers. B-3 was placed on the west side of the ASTS at the fill port area.
Figure 1 shows the locations of the borings.
Borings were advanced to a depth of ten feet. Five-foot soil core samples were collected
continuously from the direct push borings. A portion of the soil core was put in a zip-lock baggie to
allow hydrocarbons to volatilize. The air in the zip lock baggie was then screened with a
photoionization detector (PID). Laboratory samples were collected at the interval of the highest
PID reading or from just above the groundwater table. The samples were submitted for laboratory
analysis of MTBE, benzene, toluene, ethylbenzene, xylenes, naphthalene (MBTEXN), and total
petroleum hydrocarbons-gasoline range organics (TPH-GRO) using EPA Method 8260 and TPH-
diesel range organics (TPH-DRO) using EPA Method 8015.
Groundwater was encountered at approximately 4.5-6.5 feet below grade (bg). A groundwater
sample was collected from each boring using a peristaltic pump. The samples were submitted for
laboratory analysis of MBTEXN and TPH-GRO using EPA Method 8260, and TPH-DRO using
EPA Method 8015.

RESULTS
Soil Type
Soil type was evaluated during the subsurface investigation by field analysis of soil samples. The
native soil at the site consists primarily of sandy silt to five to nine feet bg. A sandy gravel layer is
below the sandy silt layer to at least ten feet bg. Boring logs are attached.
Soil Analytical Results
Table 1 lists the analytical results of the soil samples collected during the phase II investigation.
The laboratory analytical reports and chain-of-custody records are attached.
Hydrocarbon concentrations in B-1 and B-3, adjacent to the ASTs exceeded Tier 1 levels.
Concentrations exceeded Initial Screening Levels (ISLs) in the samples from B-2, adjacent to the
ASTs, and B-5, adjacent to the southeast dispenser. Samples from B-4 and B-6, at the northeast and
west dispenser were below all cleanup levels. Figure 2 shows the approximate extent of soil
contamination based on the current data. The main area of contamination is on the west side of the
ASTs, around the fill port area.
Groundwater Analytical Results
Table 2 lists the analytical results of the groundwater samples collected during the phase II
investigation. The laboratory analytical reports and chain-of-custody records for soil samples are
attached.
All groundwater concentrations were below Tier 1 levels. Samples from B-1, B-3, B-5, and B-6
exceeded ISLs. Figure 3 shows the approximate extent of contamination based on current data.
CONCLUSIONS AND RECOMMENDATIONS
Based on laboratory analytical results and field observations, the site has been impacted by releases
associated with the fuel system.
Rockwell Solutions recommends that the impacted soil around the fill ports be removed and that
monitoring wells be placed to define the extent of and monitor the natural degradation of
groundwater contamination.
