Dear Chad Phillips:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Mona Town Distribution System Improvements Project from your consultant,
Dan Fechner with Jones & DeMille Engineering on August 15, 2024. The Division issued Plan Approval for this project on April 18, 2022.Our understanding of the project is that Mona Town
has replaced approximately 3,500 feet of undersized and aging pipe with new 8-inch diameter PVC piping. This project also included the necessary isolation valves, fire hydrants and reconnecting
the existing services with new service piping.
We have received the following information for the Distribution System Project:
Certification of Rule Conformance with plan approval conditions by a professional engineer.
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Distribution System Improvementsis hereby issued as constituted
by this letter. You may now place the new distribution piping in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
