System #07062, File #15617, SRF #3F1883The Division of Drinking Water (the Division) received the plans and specifications for the subject project from your consultant, Horrocks Engineering on August 9, 2024. The project consists of the following two phases:Phase 1Install approximately 3,100 feet of 8-inch diameter C-900 DR-18 PVC water line in 36250 West and 2630 N.Install approximately 3,200 feet of 12-inch diameter C-900 DR-18 PVC water line in Tabby Swale Road Phase 2Install approximately 3,200 feet of 8-inch diameter C-900 DR-18 PVC water line in Tabby Heights Road and 4030 NorthInstall an inline booster pump on Tabby Heights Road with dual 22 gallon per minute (gpm) pumps and a 1,000-gpm fire pump. The pump station is referenced in the Divisions database as PF001. The Division has completed its review of the plans and specifications, stamped, and signed byChuck A. Richins, P.E., and dated August 9, 2024, and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, we hereby approve the proposed plans to construct the water lines and pump station, subject to the following conditions: As required by R309-550-8(8)(b)(ii)(C) and (D), verify for the Duchesne River Crossing that a means is provided (not subject to flooding) to allow for representative water quality testing on the upstream and downstream side of the crossing, and a means is provided to pressure test the underground water crossing pipe. As required by R309-550-6(6)(b), verify that the outlet of the air/vac pipe has a downward elbow and is covered by a #14 screen. As required by R309-540-5, verify that the pump station has a backup power source, and the finish floor elevation is at least six inches above the adjacent finish grade. Conditions of approval must be addressed in writing to the satisfaction of the Director when requesting an Operating Permit for the project. This conditional approval pertains to construction only. An Operating Permit must be obtained from the Director before facilities may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.The project, as described herein, is generally eligible for assistance under the Drinking Water Construction Assistance Program. All change orders will need to be reviewed and approved for compliance with the Drinking Water Rules and the conditions of this construction approval.The project may now be advertised for bids. But, as a condition of the project funding, you must not award the contract or proceed with construction until the Division of Drinking Water has authorized you to do so. This is necessary to comply with applicable requirements and to protect you, as the funding recipient, by ensuring that the contract meets requirements for funded projects. Federal regulations require that a concerted effort must be made to encourage participation of small and disadvantaged business enterprises (DBE) by providing requisite information, including plans and specifications on the project. In order to provide opportunities to DBE subcontractors, utilization goals for Minority-owned Business Enterprises (MBE) and Women-owned Business Enterprises (WBE) have been determined to be 4% and 2%, respectively. As outlined in the contract documents, the prime contractor must submit a list of prepared MBE/WBE subcontractors to be used on this project and documentation of solicitation efforts with the bid. The prime contractor's efforts to provide opportunity to DBE's must be reviewed and approved by this office before the contract can be awarded.
