Box 150545 Ogden, Utah 84415 Re: Notification of Reports Past Due Fast Stop #1116, located at 370 North Main Street, Meadow, Utah Facility Identification No. 2000340, Release Site OJT Dear Mr. Shahram: On January 10, 2024, a petroleum release from a petroleum storage tank (PST) was reported to the Division of Environmental Response and Remediation (DERR) based on the December 6, 2023, confirmation of a leak from an underground product line and removal of impacted soils during the repair work that day. On January 31, 2024, a Subsurface Investigation Reporting and Remediation compliance schedule (Compliance Schedule) was sent to you which outlined information required to be submitted to the State of Utah concerning your above-referenced Release Site. To date, the required information has not been submitted and is currently past due. Pursuant to Utah Admin. Code R311-202, Sections 280.62 through 280.65, Fast Stop is required to characterize the site by determining the extent, degree, and location of petroleum contaminated soils and the presence, concentration, and extent of dissolved petroleum contamination in the groundwater and initiate free product removal, when appropriate. The report(s) associated with these requirements is outlined in the Compliance Schedule and was granted a requested extension. The extended report was due on June 20, 2024. If the results of your overdue report indicate that laboratory analytical results are above State-established cleanup levels, or the extent and degree of the subsurface contamination is not adequately defined, you will receive and be required to comply with the requirements of additional Compliance Schedule(s). In addition, you may be required to conduct remediation (cleanup) of any subsurface contamination that exceeds applicable state-established cleanup levels. Facility ID# 2000340 Page 2 After December 31, 1995, a Certified PST Consultant must perform any PST release management, abatement, investigation or corrective action as outlined in Utah Admin. Code R311- 201-2. Failure to submit the required report(s) referenced herein within 60 days of your receipt of this notice may result in further compliance actions against you. Upon submitting your Subsurface Investigation Report, the investigation results will be evaluated to determine whether no further action (closeout) is appropriate or if additional work is necessary. This additional work may include additional investigation activities, conducting cleanup, implementing environmental monitoring and/or performing a risk assessment.
