PWS Number:UTAH18055 Deficiency Points:170 Survey Date:08/22/2024
Survey Name:MT HAVEN OWNERS
ASSOCIATION
Surveyor:Colt Smith
Sanitary Survey Category: SM
SDWIS Severity Code: Significant Deficiency
Management and Operations | DS001 | UTAH18055 DISTRIBUTION SYSTEM
24. Are there any individual home booster pumps installed in the distribution system (not for fire suppression)?
Rule: R309-550-11(3)R309-550-11(3)
Notes:There are 37 known home booster pumps with associated hydropneumatic tanks. These are
not required for pressure when the system is in operation. However, they do present a cross-
connection risk. I discussed this with Tom and Ryan and we decided this was still the most
appropriate deficiency for this system.
Deficiency Points: 50
Days until points become effective on IPS: 120
SDWIS Deficiency Description: M008 SERVICE CONNECTIONS IN DISTRIBUTION SYSTEM RELY ON INDIVIDUAL
HOME BOOSTER PUMP DUE TO INADEQUATE PRESSURE
Sanitary Survey Category: SO
SDWIS Severity Code: Minor Deficiency
Management and Operations | DS001 | UTAH18055 DISTRIBUTION SYSTEM
28A. System lacks up to 20% of required source capacity
Rule: R309-510-7(1)Rule requires a water system's source capacity to legally and physically meet the peak
day demand and the average yearly demand.
Notes:Still deficient
Deficiency Points: 15
Days until points become effective on IPS: 0
SDWIS Deficiency Description: S091 SYSTEM LACKS UP TO 20% OF REQUIRED SOURCE CAPACITY
Sanitary Survey Category: SO
SDWIS Severity Code: Significant Deficiency
Management and Operations | DS001 | UTAH18055 DISTRIBUTION SYSTEM
28B. System lacks more than 20% of required source capacity
Rule: R309-510-7(1)Rule requires a water system's source capacity to legally and physically meet the peak
day demand and the average yearly demand. This significant deficiency must be
corrected within 120 days of notification or have a corrective action plan approved by
DDW.
Notes:The system is still deficient and is working on system-specific sizing.
Deficiency Points: 50
Days until points become effective on IPS: 120
SDWIS Deficiency Description: S094 SYSTEM LACKS MORE THAN 20% OF REQUIRED SOURCE CAPACITY
Sanitary Survey Category: SM
SDWIS Severity Code: Minor Deficiency
Cross Connection and Operator Certification | DS001 | UTAH18055 DISTRIBUTION SYSTEM
38. Does the water system have detailed records of cross connection control activities? (inventories of backflow assemblies, devices, and air gaps
installed within the system)
Rule: R309-105-12(2)Rule requires each public water system to have a cross connection control program
that includes written records of cross connection control activities.
Notes:The system has written records, but they were incomplete because they did not note the
individual home booster pumps and hydropneumatic tanks.
Deficiency Points: 15
Days until points become effective on IPS: 0
SDWIS Deficiency Description: M006 CCC-LACKS WRITTEN RECORDS OF CCC ACTIVITIES
Sanitary Survey Category: SM
SDWIS Severity Code: Minor Deficiency
Cross Connection and Operator Certification | DS001 | UTAH18055 DISTRIBUTION SYSTEM
39. Does the water system have records of on-going enforcement activities? (test history, enforcement activities, hazard assessments)
Rule: R309-105-12(2)Rule requires each public water system to have a cross connection control program
that includes test history and documentation of on-going enforcement activities.
Notes:The system has written records, but they were incomplete because they did not note the
individual home booster pumps and hydropneumatic tanks, and the ongoing enforcement is
part of eliminating the potential cross connection from the booster pumps.
Deficiency Points: 15
Days until points become effective on IPS: 0
SDWIS Deficiency Description: M007 CCC-LACKS ON-GOING ENFORCEMENT IMPLEMENTATION
Sanitary Survey Category: SO
SDWIS Severity Code: Significant Deficiency
