Stericycle Transfer Facility - Traffic Impact Study

Wade,
Attached is a Traffic Impact Study that was performed for the Stericycle Transfer Facility.
Also attached is the response from the City of North Salt Lake following their review of that completed study and report.
It is my understanding that the traffic study was the major outstanding requirement in response to UDWMRC’s comments
on Stericycle’s permit application and responses to DWMRC comments.
Perhaps the next step for the permit approval process should be for the Utah DWMRC and Stericycle to again meet and
go over whether there are any additional items needed to be provided to DWMRC following the prior response from
Stericycle and this submission of the attached files regarding the traffic study.

Hi Jay,
Thank you for sending this over! There are just a few more things that need to be added to the plan of operations to meet
all the regulations:
a facility/site map, aerial photo, or blueprint
a schedule of any new construction or facility redesign
specifications on how and where daily records (weights or volumes and types of waste transferred, deviations from
plan of operation, training, inspection log) will be kept.
Once you've also added the above information to the Plan of Operations, resubmit it to us by emailing it to
dwmrcsubmit@utah.gov. The additions are pretty small and simple, so I don't think we need to have a meeting to discuss
it unless you do.
As per Brian's email, be sure to email the traffic study and city response to dwmrcsubmit@utah.gov as well. They won't be
considered officially submitted unless sent through that address.

I’ll be putting those items together and Stericycle will submit the revised Operations Plan as well as a formal submission
of the traffic study.
Just FYI …
Closure activities for the incinerator facility are progressing well: The incinerator and ancillary equipment,
including the combustion chambers, conveyors, stacks, air-pollution control systems have been removed from the
site and required samples have been taken with results below regulatory thresholds. Sump holes have been
cleaned out, filled, and resurfaced with concrete. The next major steps are to do the surface cleaning and
complete disposition of material. A report with required signature will be submitted following completion of these
closure activities.
Thanks again for your help with the Transfer Facility permit application. We should be good to proceed without meeting,
unless something unexpected come up.

R315-313-2(4)(f) buffer zone
(4) Each transfer station shall be
designed, constructed, and
operated to:
(f) have an adequate buffer
zone around the active area to
minimize noise and dust
nuisances, and a buffer zone of
50 feet from the active area to
the nearest property line in
areas zoned residential;

Utah Administrative Code
R315-302-2(2)(a) intended
schedule of construction
(2)…each plan of operation shall
include:
(a) an intended schedule of
construction. Facility permits
will be reviewed by the Director
no later than 18 months after
the permit is issued and
periodically thereafter, to
determine if the schedule of
construction is reasonably being
followed. Failure to comply with
the schedule of construction
may result in revocation of the
permit;
Provide intended schedule of
any new construction or facility
redesign.
Utah Solid and Hazardous
Waste Act 19-6-108(9)(g)(i)(ii)
Traffic Impact Study
(9) The director may not
approve a proposed
nonhazardous solid or
hazardous waste operation plan
unless the plan contains the
information that the board
requires, including: (g) for a
proposed operation plan
submitted on or after July 1,
2013, for a new solid or
hazardous waste facility other
than a water treatment facility
that treats, stores, or disposes
site-generated solid or
hazardous waste onsite, a traffic
impact study that:
(i) takes into consideration the
safety, operation, and condition
of roadways serving the
proposed facility; and
(ii) is reviewed and approved by
the Department of
Approved traffic impact study
from the agency with
jurisdiction over the roads,
including the safety, operation,
and condition of roadways
serving the proposed facility.
Transportation or a local
highway authority, whichever
has jurisdiction over each road
serving the proposed facility,
with the cost of the review paid
by the person who submits the
proposed operation plan;

R315-302-2(3)(a)(i)(ii)(iii)(iv)(v)
daily operating record
(3) … Each owner or operator
shall maintain and keep, on-site
or at a location approved by the
Director, the following
permanent records:
(a) a daily operating record, to
be completed at the end of
each day of operation, that shall
contain:
(i) the weights, in tons, or
volumes, in cubic yards, of solid
waste received each day,
number of vehicles entering,
and if available, the type of
wastes received each day;
(ii) deviations from the
approved plan of operation;
(iii) training and notification
procedures;
(iv) results of ground water and
gas monitoring that may be
required; and
(v) an inspection log or
summary; and
Specify where (or in what
database) this information will
be logged/tracked daily.
