Box 144880 Salt Lake City, UT 84114-4880 RE: Exception Report Clean Harbors Aragonite, LLC EPA Number -UTD 981 552 177 Dear Mr. Hansen, In accordance with Module 1 condition 1.A.3 of Clean Harbors Aragonite's (CHA) State RCRA Permit, 40 CFR 262.42, 40 CFR 761.217 and R315-262-42, this correspondence serves as written notification of one (1) shipment of hazardous waste or PCB regulated waste to other facilities that CHA has not received a signed copy of the received manifest from the destination facility. The manifest information and efforts CHA are taking to resolve each issue can be seen below: • Manifest 019497654FLE -CHA has tracked this manifest, and it is still on route to the destination facility. It has been delayed due to several backlogs. A copy of the manifest has been attached. I certify under penalty of law that this document and all attachments were prepared under my direction or supervision according to a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete.
