Attn: Rick Lybbert, Registered Agent 166 Cottonwood Loop Saratoga Springs, Utah 84045 Subject:Operating Permit, Last Chance Ski Ranch Well (WS001) Storage Tank (ST001) Booster Pumps (PF001) Arsenic Removal Plant (TP001) and Distribution System (DS001); Last Chance Ski Ranch, System #23088, File #7567, #7579 Dear Rick Lybbert:The Division of Drinking Water (the Division) issued Plan Approval for the WS001 Well Equipping on November 10, 2008, File #7579. Plan Approval was issued for the ST001 Storage Tank, PF001 Booster Pumps, TP001 Arsenic Removal Plant, and DS001 Distribution System on December 1, 2008, under File #7567. Multiple site visits have been conducted to the facilities by Division Staff. Final inspection was completed on July 26, 2024, by Brian Pattee and Brent Arns. Our understanding of the system is: Last Chance Ski Ranch Well (identified as WS001 in the Division’s database): The well is a six-inch diameter steel casing installed to an approximate depth of 191 feet. The well was drilled using a Bucyrus Erie cable tool rig and consists of a 10-inch diameter temporary surface casing drilled to 100 feet. The bore hole (11-inch diameter drive shoe) extends down to a total depth of 190 feet with 6-inch diameter steel blank casing and 6-inch diameter machine slotted intake casing extending down to 184 feet inside the temporary 10-inch casing. Blank casing was placed from +18 inches down to 110 feet and 6-inch slotted casing between 110-150 feet (slot size is .125" at 228 slots per 20 linear foot length of casing) with a total of 456 intake slots from 110 to 150 feet. The pump installed is a 0.75 HP submersible pump with a pitless adaptor. The listed well pump capacity is 17.6 gallons per minute (gpm). The safe yield of the well is rated at 11.3 gpm, which is calculated based on two thirds of the constant-rate well pump test results at l7 gpm. Last Chance Ski Ranch Storage Tank (identified as ST001 in the Division’s database): The tank is a square concrete storage tank with an effective volume of 8,000 gallons. Last Chance Ski Ranch Culinary Booster Pump Station (identified as PF001 in the Division’s database): The Booster Pump Station consists of a hydropneumatic tank with one duty pump and one backup booster pump. Last Chance Ski Ranch Arsenic Removal Plant (identified as TP001 in the Division’s database): The arsenic removal plant consists of an adsorptive media arsenic removal system make Layne SolmeteXwater, model npXffa 14 system with ArsenX hydrous iron oxide media. Last Chance Ski Ranch Distribution system (identified as DS001 in the Division’s database): The distribution system consists of 19 connections, and culinary distribution piping. Fire hydrants and fire suppression is NOT supplied and or connected to the culinary distribution system piping. We have previously received the following information for Last Chance Ski Ranch Water System: Certification of Rule Conformance with plan approval conditions by a professional engineer (Matt W. Brown, P.E.) As-built or record drawings. Evidence of O&M manual delivery. Copy of the order from the State Engineer for permanent change, application number l5-3080 (a33654) for the valid water right. Satisfactory bacteriological results from the disinfection of the well, treatment system, storage tank. booster pump. and distribution system. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Last Chance Ski Ranch Well (WS001) Storage Tank (ST001) Booster Pumps (PF001) Arsenic Removal Plant (TP001) and Distribution System (DS001)is hereby issued as constituted by this letter. You may now place Last Chance Ski Ranch Well (WS001) Storage Tank (ST001) Booster Pumps (PF001) Arsenic Removal Plant (TP001) and Distribution System (DS001) in service in your water system. Please maintain a copy of this letter with your permanent records for future reference. Drinking Water Source Protection Concurrence The Preliminary Evaluation Report (PER) for the Last Chance Ski Ranch Well was approved in the Well Drilling Approval letter dated May 22, 2008. The Drinking Water Source Protection (DWSP) plan was submitted and concurred with by the Division on November 9, 2017. An updated DWSP plan was submitted on June 21, 2024, and is currently pending review. Issuance of this operating permit does not change this system's monitoring and reporting requirements. Pursuant to R309-215-6(2)(a), the Arsenic Removal Plant (TP001) shall be sampled quarterly for Arsenic.
