(Anfield), Shootaring Canyon Mill, Garfield County, Utah Inspection Items: Radiological Posting, Notice to Workers Postings, External Monitoring, Radiation Safety Training and Dosimetry Inspection Date: June 12, 2024 Inspector: Ryan Johnson, Utah Division of Waste Management and Radiation Control (DWMRC) Personnel Contacted: LaMiya Morrill, Site Supervisor DWMRC Meters Used Model Serial Number Calibration Due Date Dose Rate Bicron 9048 9/18/2024 Note: Instruments were source checked prior to inspection. Governing Documents: Radioactive Materials License UT0900480 UAC R313-15 UAC R313-18 UAC R313-24 Opening Meeting Anfield Personnel LaMiya Morrill, Site Supervisor DWMRC Personnel Ryan Johnson, DWMRC Inspector The inspector briefly discussed the purpose of the inspection with Anfield’s Site Supervisor. The inspector discussed with them that the inspection would be a tour of the Mill and a review of records. Inspection Summary The inspection consisted of a tour of the Shootaring Canyon Mill facilities and reviewing applicable documents. Due to the inactive status of Shootaring Canyon Mill, no work activity was observed within the Mills restricted area. 2 of Page 3 U:\RAD\COMMON\Uranium mills\UT0900480 Anfield Res - Shootaring Cyn UMill\HP Inspection Modules\2024\RADMOD-Shootaring Inspection Report 6-12-2024.docx Inspection Item Mill Tour:Anfield staff took the inspector on a tour of the Shootaring Canyon Mill’s facilities. The inspector observed: The entrance to the Mill and the gates to the Mills restricted area were properly posted. Postings around the Mill were legible and in good condition; The required Notice to Workers documentation (as required by UAC R313-18) on the wall in the break room of the facilities office building; All of Anfield’s personnel wore proper dosimetry within the restricted area of the Mill and their dosimetry was kept in the Mill’s office building when not being used; Proper radiological instrumentation was being used, maintained and calibrated; Personnel surveyed themselves before leaving the restricted area. Perimeter fencing and gates were in good condition; Known and suspected contaminated areas within the restricted area. Discussions of these areas were about what needed to surveyed, cleaned up and documented prior to any work being done within the restricted area; Ore on the ore pad was covered with soil; Radiological general area dose rate measurements taken by the inspector during the inspection and are as follows: o 8-20 µrem/hr inside the restricted area; The following documents were reviewed by the Inspector: The Radiological training records for the Mill’s Supervisor and Staff and found them to be complete, legible and appropriate; and The facility’s ALARA Report and determined it met the requirements of the License. Deficiencies: None Other: During the records review, the Licensee provided a written memo documenting minor self- reportable items. They are as follows: 1. The RSO had the tape under the leach tanks changed from saying “Radiation Area” to a caution tape with small signs attached that said “Any Area within this area may Contain Radioactive Material”. 2. A survey meter was used past its calibration due date for about one month. When it was discovered, the Licensee replaced the meter with a calibrated meter and the out-of-date meter was sent in for calibration. The inspector determined that the 1st item is not a compliance issue because the caution tape was placed there to discourage people from walking around the leach tanks and through the sandy material that is under them. The 2nd item is a minor compliance issue and would be a severity level IV. The inspector determined that the Licensee acted appropriately and the corrective actions in the written document will be sufficient. The memo can be reviewed by request at the Mill facility.
