HomeMy WebLinkAboutDDW-2024-010024 August 15, 2024 Joshua Bake Roosevelt City Water System 255 South State Roosevelt, Utah 84066 Subject:Operating Permit, 800 South water line replacement, System #07004, File #11095The Division of Drinking Water (the Division) received a request for an Operating Permit for the subject project from your consultant, CIVCO Engineering on August 1, 2024. The project consisted of replacing the existing water line in 800 South from State Street to 500 East with approximately 1,700 feet of 12-inch diameter C905 SDR 21 PVC water line and associated appurtenances. The Division issued Plan Approval for the project on January 8, 2018. The Operating Permit request included a certification by a professional engineer, record drawings and satisfactory bacteriological results as evidence of proper disinfection and flushing. The Division has determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the 800 South water line replacement projectis hereby issued as constituted by this letter.Please maintain a copy of this letter with your permanent records for future reference.
