The Report details the groundwater monitoring well installation and first quarter, 2024 sampling performed at the 21st & 21st Properties (Site) located at 2100 South 2100 East, Salt Lake City, Utah. Comments pertaining specifically to the Report are included in the attachment to this letter. Please provide a response to the attached comments by July 15, 2024. In addition to the attached specific comments on the Report, the Division has identified several other issues related to the Site. 1.The Division understands that a Corrective Measures Implementation Report (CMIR) is in development, outlining the completed tasks specified in the March 2021 Remedial Action Plan (RAP). The Report indicates in Section 5.0 that the Vapor Intrusion Mitigation System (VIMS) was installed at the Site, the source was removed, and contaminated soil was excavated and taken off-site in accordance with the RAP. As of the date of this letter, the Division has not received the CMIR and cannot verify the actions described in the Report. Please provide the CMIR by July 1, 2024. (Over) Page 2 of 3 2.It is the Division’s understanding that the Revised Site Management Plan (SMP) Outline (Outline) submitted February 17, 2022 (and as provided in Appendix D of the Report), outlines the installation of monitoring wells and monitoring requirements for the Site. Additionally, the Outline highlights the steps to be taken if contaminant action limits are exceeded and if commercial land use exceedances are identified. According to Section 5.0 of the Report, data from all on-site monitoring wells (MW-1A, MW-4, and MW-5) exceeded the U.S. Environmental Protection Agency (EPA) Commercial Vapor Intrusion Screening Level (VISL) for tetrachloroethene (PCE). Additionally, the monitoring well on the off-Site property and to the west of the Site (MW-2A) exceeds the U.S. EPA Commercial VISL for PCE as well. Based on the agreements set forth in the Outline, additional investigation is required. The Division is aware that the installation of the VIMS was proposed in the RAP. However, the Outline indicates that if a Cancer Risk of 1E-04 or greater and a hazard index of 1 or greater are identified in on-site monitoring wells, “Terracon must prove that the VIMS is protective for this risk range.” Per the Outline, the following is to be performed for four consecutive quarters to ensure the VIMS is adequately mitigating the vapor intrusion pathway: a.The VIMS exhaust pipes should be sampled for the Contaminants of Concern (COCs) that include PCE, trichloroethylene (TCE), cis-1,2-dichloroethene (cis-DCE), 1,1- dichloroethylene (1,1-DCE), and vinyl chloride. b.Permanent sub-slab vapor points should be installed and sampled for COCs. c.Indoor air sampling for COCs should be performed in tandem with the sub-slab vapor sampling. Evaluation of the efficacy of the VIMS must address cumulative risk to the commercial receptor, not just individual contaminant risk. The Report indicates the Commercial VISL for PCE was exceeded at MW-2A. This exceedance triggers additional investigation of potential off-stie impacts per the Outline. Soil gas sampling, in accordance with the Outline, must be performed to assess whether COCs pose a potential risk to the property associated with MW-2A, and areas west of the Site. Please provide the Additional Investigation Work Plan (Work Plan), assessing contamination and exposure pathways at the Site as described in the Outline by August 1, 2024. (Email) Tom Henroid, Rockworth Companies, LLC (Hard Copy and Email) Page 1 of 3 Division Comments on the Monitoring Well Installation and Quarterly Groundwater Monitoring Report, April 2024 1.Throughout the Monitoring Well Installation and Quarterly Groundwater Monitoring Report, April 2024 (Report) for the 21st & 21st Properties (Site) located at 2100 South 2100 East, Salt Lake City, Utah, reference is made to MW-1 through MW-3. As noted in Section 1.1 of the Report, MW-1 through MW-3 had production issues and replacement wells MW-1A through MW-3A were installed in January 2024. The Report does not refer to the replacement well numbers, which creates confusion as to which wells were sampled. Revise the Report in its entirety to refer to the correct well numbers, e.g., MW-1A, MW-2A, and MW-3A, consistent with the well logs in Appendix C. 2.Section 1.1 of the Report indicates that new monitoring wells were constructed due to the inability to sample the original monitoring wells on-Site (MW-1, MW-2, and MW-3). However, the Report does not indicate whether the original wells were abandoned or destroyed before or during the redevelopment of the Site. Update the Report to indicate the status of the original three monitoring wells. If the monitoring wells are still active update the figures to include their location. 3.Section 1.1 indicates that the Outline was submitted to Dale Urban on February 17, 2024. Please clarify the text that the Outline was provided in 2022. 4.The installation of five monitoring wells (MW-1A, MW-2A, MW-3A, MW-4, and MW-5) was proposed in the Revised Site Management Plan (SMP) Outline (Outline). However, the construction details were not expressly included in the Outline nor submitted in a formal work plan. As such, it is not clear whether details on well construction were discussed with Division personnel. Section 1.2 of the Report outlines the monitoring well installation and construction, which includes all five wells installed to approximately 40 feet below land surface (ft bls) and screened from 20 to 40 ft bls. These screen lengths are atypically long. Several uncertainties arise with the increase in monitoring well screen length, most notably contamination location and mobilization through the lithology. Please revise the Report to include the rationale of the chosen well construction methodology and to discuss uncertainties in comparing data collected across varying well screen lengths. 5.Section 1.2 indicates the newly installed monitoring wells were developed using a surge block and bailer until geochemical parameters were stabilized, with a minimum of six “purge volumes” extracted from each monitoring well. During the Division’s sampling observation on March 31, 2024, the purge water from the observed wells appeared highly turbid and it was unclear how stabilization was determined, based solely on observation of the color of the groundwater sample. The stabilization criteria used to determine if the monitoring wells were fully developed were not discussed in the Report. Additionally, “purge volumes” and the total development volume are not detailed in the Report. Please discuss what parameters and stabilization criteria were used for development. Additionally, define purge volumes and include the well development logs in the Report, so calculations may be verified. Page 2 of 3 6.Sections 1.2 and 2.1 indicate that purge water from the development and sampling of new wells was spread on-site and allowed to infiltrate into the ground. Division does not practice thin spreading without generator knowledge and/or waste characterization data. Additionally, if generator knowledge is known and thin spreading is practiced, the water extracted from the wells should never be allowed to leave the Site or infiltrate into the ground. Thin spreading should be performed on an impervious surface to allow known contaminants of concern to volatilize without being reintroduced into the subsurface. Please provide justification, e.g., historical waste analysis data, for handling investigation derived waste in this manner. 7.Section 1.3 of the Report states MW-4 was unable to be surveyed due to “considerable large debris located on top of the well.” As the well was installed in January 2024, it is unclear the source of this debris. Please indicate if MW-4 will be surveyed and when the survey will occur so that it may be included in future groundwater elevation measurements. The Division understands that the survey of the monitoring wells was collected off an “arbitrary semi-permanent on-site benchmark.” If this method is used to survey monitoring well elevations, MW-4 should be measured based on the same benchmark as the monitoring wells that were previously surveyed. If the same benchmark cannot be utilized, it is recommended that all monitoring wells on-site be resurveyed. 8.Section 2.1 of the Report outlines the sampling procedure used to sample all five monitoring wells at the Site. The Report indicates the monitoring wells were purged and sampled using a bailer. However, there are no decontamination procedures discussed in the Report for how the equipment used to sample was decontaminated in between monitoring wells. Update the Report to discuss the decontamination procedures and efforts to reduce cross-contamination for the water level meter and bailer used during sampling. If a new bailer and twine was used for each monitoring well, please update the Report accordingly. 9.The Report outlines the use of a bailer to sample the monitoring wells. During the Division’s observation of sampling on March 31, 2024, Terracon personnel stated that the groundwater was too deep to perform low-flow sampling using a peristaltic pump. The Division suggests sampling via low-flow sampling procedure to obtain the most accurate, representative groundwater sample possible at the Site. Please update the Report with rationale of using a bailer rather than a peristaltic pump for this sampling event and the planned sampling procedure for future groundwater monitoring events using the low-flow method. 10.Section 2.1 of the Report states that prior to sampling, geochemical parameters (temperature, pH, specific conductivity, dissolved oxygen, and oxygen reduction potential) were collected until stability was achieved or three volumes were purged from the monitoring wells. However, the Report does not define what groundwater parameter values are used to determine stabilization or what constitutes a “volume” (i.e. well volume, boring volume, equipment volume). Please provide the well volume calculations and groundwater sampling forms demonstrating stability of groundwater geochemical parameters or sufficient well purge volumes. 11.Section 2.2 indicates groundwater elevations ranged from 67.22 to 77.22 feet above mean sea level. The Division understands that a survey was performed using an arbitrary benchmark rather than a benchmark with a known elevation above mean sea level. Based on the relative elevation of the Site, the groundwater elevations are not likely representative of actual feet above mean sea level and should be reported accordingly throughout the report, in the figures, and in Table 1. Page 3 of 3 12.Section 5.0 indicates that the off-Site well, MW-2A, exceeds the commercial vapor intrusion screening level (VISL) for tetrachloroethylene (PCE). As noted in Exhibit 2, MW-2A is located near a building (northwest corner of 2100 S and 2100 E). It appears that the extent of the groundwater plume has not been defined in this direction. Discuss the potential for vapor intrusion issues in this building. Also clarify if the owner of the building has been notified of potential vapor intrusion issues due to impacted groundwater underlying the building. 13.The Outline provided in Appendix D indicates the on-site monitoring action levels are based on a cancer risk of 1E-04 or greater and a hazard index of 1 or greater. However, in order to enter into an SMP, the total (cumulative) cancer risk must be shown to be within the acceptable cancer risk range of 1E-04 to 1E-06 and equal to or below a hazard index of one, as defined in Utah Administrative Code R315-101. While use of the action levels (VISLs) provides a gauge as the magnitude of residual contamination, individual comparison is not acceptable, but rather total risk and hazard must be determined. 14.The Division of Environmental Response and Remediation (DERR) Leaking Underground Storage Tank (LUST) located in the southwest portion of the Site was discussed in Section 5.0 of the Report. The DERR LUST regulatory requirements and regulations do not fall under the Division’s Environmental Cleanup Program (ECP) and are not governed under the requirements of Utah Administrative Code R315-101 which establishes the requirements for the Sites under in the ECP. As such, the DERR LUST at the Site should not be included in the Report. Additionally, DERR initial screening levels (ISLs), Tier 1 screening levels, and Utah groundwater quality standards should not be listed in the Tables or used to scrutinize analytical results as they are not relevant to Utah Administrative Code R315-101. Please update the report accordingly. Responses to DWMRC May 30, 2024, Comments on the Monitoring Well Installation and Quarterly Groundwater Monitoring Report, April 2024 1. Well Number Updates Replacement well numbers MW-1A, MW-2A, and MW-3A now included. 2. Previous Well Abandonment Abandonment procedures provided in the report. These wells are no longer active. 3. Outline Submittal Updated. 4. Well Construction Details The well construction details were discussed on multiple occasions with Dale Urban. It was decided to go to 40 ft bgs to ensure that we were well within the groundwater column to properly sample, especially since suspected DNAPL may be present. Previously, well depths to 30 ft bgs produced little to no water. Groundwater can fluctuate on and off-site due to high influx years and clayey layers present at depth, and we wanted to make sure that water would not go above the screening interval (especially since there was also historically LNAPL at a portion of the site). Had we screened a smaller interval (i.e. from 40-25 ft bgs), both MW-1A and MW-4 would have groundwater above the screened zone, which can lead to considerable uncertainties. A 20-foot screening level is rather common. 5. Well Development Development forms are included in Appendix E and described in the report. 6. Purge Water Any potential source has been previously removed from the site as determined by substantial stockpile and soil confirmation conducted at the site. No significant off-site soil or groundwater exceedances in chlorinated solvents or petroleum hydrocarbons had previously been noted. Additionally, any groundwater generated during purging (both off and on-site) was placed in portions of the site where off site migration would not be predicted, and a vapor barrier is present under the residential buildings at the site. Due to the results obtained during this investigation, any water purged during future well sampling activities will be drummed and characterized as needed. The installation, development, and sampling of both these wells and previous wells was discussed with Dale Urban on multiple occasions. Well installation, development, and sampling occurred on and off-site in 2021. It was never requested that any water generated be drummed, and no comment was received from DWMRC requesting this to occur before or after the sampling event or the report being issued. 7. Well Survey of MW-4 The same benchmark will be utilized to survey MW-4 once the location is accessible. The area is currently an active construction zone and covered in 10-20 foot long steel I-beams being used for outer building construction that began after our initial well installation. 8. Decontamination Procedures Updated and addressed in the report. 9. Low-Flow Sampling Although not required, it was originally planned to conduct low-flow sampling techniques to sample these wells. Due to the depth of the groundwater (approximately 25 feet bgs), peristaltic pumps are not able to effectively pump from that depth and still provide a stable flow to accurately sample these wells. Purging at least three well volumes and sampling using a bailer was then utilized instead, which is also an acceptable method. 10. Sampling Parameters Report has been updated to include sampling and parameter notes. 11. Arbitrary Benchmark Report and exhibits have been updated. 12. Extent of Groundwater Plume and Vapor Intrusion Issues A previous groundwater sample collected at former monitoring well MW-2 did not exceed a VISL for a chlorinated solvent. Additionally, a groundwater sample collected further west of the off-site building during a previous investigation did not exceed a regulatory screening level. Due to a single event showing a regulatory exceedance, it is premature to state that there is a potential vapor intrusion issue off-site. Additionally, a risk evaluation was conducted at this location with the concentrations of chloroform, PCE, and TCE imputed. The calculated carcinogenic risk was 4.48 x 10-5, which is below the 1 x 10-4 target commercial hazard index. It was recommended in the current groundwater monitoring report to conduct at least one more round of groundwater sampling to determine whether this recent reading is anomalous or if a potential issue is present. Another quarterly groundwater monitoring event is expected to occur in June 2024. If the calculated carcinogenic risk exceeds the commercial hazard index, we will propose an additional investigation as needed. 13. Action Levels An extensive VIMS system was designed and installed at the site. This groundwater report is intended to present the results from the most recent groundwater sampling event. Another quarterly groundwater monitoring event is scheduled for June 2024. As discussed in the recent report, we would like to determine whether concentrations found during this sampling event are anomalous or further investigation is warranted. Previous data collected at the site does not indicate the same level of concern as this recent data. 14. DERR LUST Screening Levels The southwest portion of the site is a DERR regulated LUST site. This report is also submitted to Eileen Brennan, by request, in order to request closure for that related portion of the site. DERR results are applicable for the LUST portion of the site and, hence, should remain in the report. 6952 South High Tech Drive Ste. Numerous Phase I Environmental Site Assessments and subsequent Limited Site Investigations were conducted at the properties from approximately 2014 through 2019 by Terracon and others. Results of these studies indicated there were soil and groundwater impacts from chlorinated volatile organic compounds (VOCs) thought to have originated at on-site historical dry cleaning businesses as well as from off-site sources. In 2020, two additional parcels were added to the proposed development: the Blue Plate Diner parcel and the 21st Coffee parcel. Two historical dry cleaners and an historical gas station had operated at these properties, and these former businesses were found to have contributed to the chlorinated VOC impacts. In addition, the historical gas station was identified as a release site and was issued an Underground Storage Tank (UST) facility ID and a Release ID. The ten parcels were re-apportioned into one parcel and assigned the APN of 16-15-359-018-0000 and street address 2145 East 2100 South, which is now the Site. Investigations conducted at the site are detailed in the Remedial Action Plan (RAP, Terracon, April 7, 2021) and are thus included in the public record. Three Amendments to the RAP were also submitted detailing changes to soil and stockpile sampling methodology and further characterization of specific areas of the site. A Limited Site Investigation Report was submitted by Terracon on March 2, 2021, that detailed the installation and sampling of three monitoring wells (MW-1 through MW-3) on and off-site in order to determine whether there are impacts to soil and/or groundwater relating to the historical on-site LUST facility and to monitor the potential impacts over time. The monitoring wells were planned to be drilled to approximately 30 feet bgs, but MW-3 was only drilled to 26.5 feet bgs due to refusal. The wells were later developed, surveyed, and sampled. Groundwater from MW-1 had detections of tetrachloroethylene (PCE) and trichloroethylene (TCE) above United States Environmental Protection Agency (EPA) Maximum Contaminant Levels (MCLs) and PCE above the EPA commercial Vapor Intrusion Screening Level (VISL). Groundwater from MW-2 detected PCE above the EPA MCL and EPA residential VISL. None of the borings encountered soils with chlorinated compounds above EPA Regional Screening Levels (RSLs). Monitoring Well MW-3 could not be developed or sampled as it never yielded groundwater. Monitoring Well MW-1 and MW-2 only periodically yielded enough groundwater to accurately sample, and recharge was slow. Additionally, it was likely that MW-1 would eventually need to be abandoned due to planned construction of a retail and residential building over that location. Monitoring Well Installation and Report 21st and 21st Salt Lake City, Utah June 7, 2024 | Terracon Project No. 61197460 Facilities | Environmental | Geotechnical | Materials 2 A Revised Site Management Plan Outline was submitted to then DWMRC Project Manager, Dale Urban, on February 17, 2022, as part of the continuing discussions concerning post- construction site management and groundwater monitoring (Appendix D). The memo detailed the construction of two new on-site monitoring wells (MW-4 and MW-5), one on- site replacement monitoring well (MW-1A), and two offsite replacement monitoring wells (MW-2A and MW-3A). The replacement wells MW-1A through MW-3A would be drilled to deeper depths in order to ensure adequate groundwater recharge and accurate sampling and be consistent with the new wells being constructed at the site. The memo also discussed five years of groundwater monitoring with the first year consisting of quarterly sampling events. Exhibit 1 (Appendix A) presents a topographic map of the area. Exhibit 2 (Appendix A) is a site diagram that shows the monitoring well locations. The replacement wells are labeled as MW-1A, MW-2A, and MW-3A. 1.2 Soil Boring and Monitoring Well Installation The original monitoring wells MW-1, MW-2, and MW-3 were first abandoned properly by removing the well box. The 2-inch diameter PVC pipe was then filled with hydrated bentonite chips. The well casing was then broken and the surface was restored to its original condition (concrete, asphalt, or native soil). Since these wells did not exceed 30 feet bgs, they were not subject to permitting under the Division of Water Rights. Prior to monitoring well (new and replacement) drilling and installation, the public utility location service (Blue Stakes of Utah) was notified at least 48 hours prior to digging or drilling as required by law. In addition, a subcontracted private utility location service, Earthview Technology Inc., was used to identify and mark subsurface utilities around each well location. A Non-Production Well Application was filed with the State of Utah Division of Water Rights, which is required for any monitoring well that extends below 30 feet bgs. The groundwater monitoring wells were drilled by Cascade Environmental from January 9, 2024, to January 12, 2024, using a limited-access sonic drill rig. During drilling, the soil was logged and field-screened using sensory methods and a photoionization detector (PID) to detect the presence of VOCs. Please refer to the soil boring and monitoring well construction logs in Appendix C. The monitoring wells were drilled to an approximate depth of 40 feet bgs. The monitoring wells were constructed as follows: Installation of 20 feet of 2-inch diameter (20-40 feet bgs), 0.010-inch machine slotted polyvinyl chloride (PVC) well screen with a threaded bottom cap. Installation of 2-inch diameter, threaded, flush-joint PVC riser pipe to the surface. Addition of pre-sieved silica sand pack for annular sand pack around the well screen from the bottom of the boring to approximately 2 feet above the top of the well screen. Monitoring Well Installation and Report 21st and 21st Salt Lake City, Utah June 7, 2024 | Terracon Project No. 61197460 Facilities | Environmental | Geotechnical | Materials 3 Placement of hydrated bentonite chips above the sand pack to just below the well box (approximately one-foot bgs). Installation of a 7-inch diameter, circular, bolt-down, steel, monitoring well cover with a locking well cap inset in a flush-mount concrete well pad. After installation and prior to sampling, the wells were developed using a surge block and bailer. Development continued until parameters (pH, temperature, dissolved oxygen (DO), and conductivity) stabilized to have consecutive readings be within approximately 10% or the when the well went completely dry multiple times after multiple bailing events. At least nine purge volumes (calculated volume of water present in the water column) were extracted from each monitoring well. Well development forms are provided in Appendix E. Soil cuttings collected during drilling were placed in 55 gallon drums and picked up and disposed of by E.T. Technologies, Inc. Purge water collected during well development and later sampling was spread onsite and allowed to infiltrate into the ground. 1.3 Groundwater Elevation Survey The elevation of the tops of the well casings were surveyed relative to an arbitrary semi- permanent on-site benchmark. Horizontal locations were determined via an Arrow GPS unit with sub-meter accuracy. Groundwater levels were measured in the monitoring wells prior to groundwater sampling. The measured water levels were used in combination with the casing elevations to evaluate groundwater elevations, direction of groundwater flow, and gradient. On-site monitoring well MW-4 was unable to be properly surveyed due to considerable large debris located on top of the well. Exhibit 3 (Appendix A) indicates the approximate groundwater gradient and flow at the site. Based on the most recent data collected during this sampling event, groundwater flows to the southwest across the site consistent with historical studies. 1.4 Scope of Services The work was to install five new groundwater monitoring wells (three wells replacing older wells) and collect groundwater samples to assess the current groundwater conditions. This is the first of four quarterly groundwater sampling events planned for the site. 1.5 Standard of Care Terracon’s services were performed consistent with generally accepted practices of the profession undertaken in similar studies in the same geographical area during the same time period. Please note that Terracon does not warrant the work of laboratories, regulatory agencies, or other third parties supplying information used to prepare the report. These services were performed in accordance with the scope of services agreed with you, our client, as reflected in our proposal, and were not restricted by ASTM E1903-19. Such information is subject to change over time. Certain indicators of the presence of hazardous substances, petroleum products, or other constituents may have been latent, inaccessible, unobservable, non-detectable, or not present during these services, and we cannot represent that the site contains no hazardous substances, toxic materials, petroleum products, or other latent conditions beyond those identified during this investigation. Subsurface conditions may vary from those encountered at specific borings or wells or during other surveys, tests, assessments, investigations, or exploratory services; the data, interpretations, findings, and recommendations are based solely upon data obtained at the time and within the scope of these services. 1.7 Reliance This report has been prepared for the exclusive use and reliance of Rockworth Companies and the Utah Department of Environmental Quality (UDEQ). Use or reliance by any other party is prohibited without the written authorization of the Rockworth Companies and Terracon. Reliance on the report by the client and all authorized parties will be subject to the terms, conditions, and limitations stated in the proposal, report, and Terracon’s Terms and Conditions. The limitation of liability defined in the Terms and Conditions is the aggregate limit of Terracon’s liability to the client and all relying parties. 2.0 Groundwater Sampling Event 2.1 Groundwater Sample Collection On March 21, 2024, Terracon collected groundwater samples from the three onsite and two offsite monitoring wells (MW-1A, MW-2A, MW-3A, MW-4, and MW-5). Prior to collecting the groundwater samples, each well was purged using a single-use disposable plastic bailer and twine which was discarded after use on each well. The water meter used for depth to water measurements was decontaminated using an Alconox wash followed by a rinsing with potable and distilled water. Parameters, including temperature, pH, conductivity, DO, and oxygen reduction potential (ORP) were periodically collected during the purging. Each well was purged a minimum of three well volumes (calculated volume of water within the initial water column) prior to sampling. No sheen or distinct odor was noted during the sampling. Purge water was thin spread across the surface of the site and allowed to evaporate or infiltrate into surface soils. Appendix F includes the field notes associated with the groundwater sampling event. 2.2 General Observations Depth-to-water measurements and relative groundwater elevations are presented in Table 1 (Appendix B). Relative groundwater elevations at the site ranged between 67.22 (MW-3) and 77.22 (MW-5) feet above an arbitrary established benchmark. The groundwater gradient at the site was determined using ArcGIS Pro and scaling the area in order to interpolate locations of contour lines to show the gradient. The direction of groundwater flow is generally toward the southwest (Exhibit 3, Appendix A), consistent with historical data. Free product was not observed in any monitoring wells during this sampling event. 3.0 Laboratory Analytical Methods All groundwater samples were collected and placed in laboratory-prepared containers, labeled, placed on ice in a cooler, and transported to Pace Analytical National Center for Testing and Innovation (Pace Analytical) located in Mount Juliet, Tennessee. The groundwater samples were submitted for analysis of VOCs using EPA Method 8260B. The executed chain-of-custody form and laboratory data sheets are provided in Appendix H. 4.0 Data Evaluation 4.1 Groundwater Sample Results Results of the groundwater sampling event are summarized in Table 2 (Appendix B). Sample locations are shown on Exhibit 2 (Appendix A). As presented in Table 2 (Appendix B), the following VOCs exceeded a regulatory screening level. Residential VISL exceedances of TCE, chloroform, and 1,1,1,2-tetrachloroethane were reported in at least one well. The offsite well locations are current commercial businesses. 4.2 Quality Control Quality control for the sampling program was provided by using standardized sample collection and handling methods, documenting pertinent field information, and keeping chain-of-custody records, as well as using a Utah-certified laboratory. All sampling procedures followed Terracon’s Standard Operating Procedures and industry practices. A review of the field procedures indicates that standard operating procedures were followed with regards to sample collection, handling, and documentation. The analytical laboratory report indicated that samples were extracted and analyzed within appropriate sample holding times. Internal laboratory QA/QC procedures and results are presented in the laboratory report and were within acceptable limits set by the laboratory, except where noted. 5.0 Findings And Conclusions On-Site Monitoring Well Results PCE in groundwater is elevated at the site above Residential and Commercial VISLs. Concentrations of PCE in groundwater were considerably higher than historical groundwater samples taken in the vicinity of MW-1A, MW-4 and MW-5. TCE is also elevated above Monitoring Well Installation and Report 21st and 21st Salt Lake City, Utah June 7, 2024 | Terracon Project No. 61197460 Facilities | Environmental | Geotechnical | Materials 7 Residential VISLs in select areas. A Vapor Intrusion Mitigation System (VIMS) was installed at the site to mitigate potential vapor intrusion associated with the elevated PCE in groundwater. Extensive soil removal has occurred at the site, and any possible on-site source of the contamination has been removed. If the site is considered a source of the PCE contamination, it is expected that the PCE concentrations in these onsite wells will decrease over time. Minor construction of the VIMS system and vapor barrier is still being conducted at the site. Vent pipe construction has not been full completed, and the pipes have not been fully tested. Additionally, commercial and residential tenants have not moved into the site. Terracon recommends conducting an additional round of quarterly groundwater sampling (scheduled for June 2024) to determine whether a further investigation is warranted. A former gas station was located in the southwest portion of the site, near monitoring well MW-1A. The groundwater in this area does not appear to be contaminated with petroleum- related hydrocarbons (benzene, toluene, ethylbenzene, xylenes, naphthalene) above regulatory screening levels. The contaminated soil has been excavated and removed in this area to the extent practicable. If continued groundwater monitoring indicates the absence of these analytes above regulatory screening levels, closure of the DERR LUST portion of the site should be considered. Off-Site Monitoring Well Results Off-site well MW-2A exceeded the Commercial VISL for PCE. Results from this sampling are considerably higher than concentrations from the previous sampling event at the initial adjacent well location, which did not exceed the Commercial VISL. A risk evaluation was conducted on off-site monitoring well MW-2A using the EPA Regional Screening Level (RSL) calculator to evaluate if the cumulative risk posed by chemicals remaining in the off-site groundwater represent a human health risk at levels above target maximum risk levels and hazard indices. A hazard quotient of 1.0 was used and a target risk of 10-6 was selected to be conservative. Although this site is commercial, the scenario was run as “Resident.” The concentrations of PCE, TCE, and chloroform were imputed. The results are presented below and shown in Appendix G. The cumulative carcinogenic risk posed by the remaining chemicals in the groundwater off- site is 4.48 x 10-5, which is above the target excess residential cancer risk of 1 x 10-6 but below the excess commercial residential cancer risk of 1 x 10-4. The area around MW-2A is developed for commercial use. Terracon recommends monitoring this location for the next several quarters in order to determine whether contaminant concentrations are stable and decreasing. A risk evaluation will be run after each sampling event, and further investigation will be considered (i.e. soil gas investigation) if carcinogenic commercial land use exceedances are determined. Terracon has expanded upon the discussion held to compose an outline of a scope of work associated with the site management plan as discussed. It is our understanding that once corrective action with controls is implemented (i.e., the Remedial Action Plan (RAP) is implemented fully, a Site Management Plan (SMP) including a Vapor Intrusion Mitigation System (VIMS) design is composed and approved, and environmental deed restrictions are recorded), a letter acknowledging that corrective action has been completed can be issued by the DWMRC. On-going monitoring will be encompassed as part of the SMP, which is performed post corrective action. It was established during our discussion that the purpose of groundwater monitoring in the SMP is to measure the results of the remedial action performed, which consisted of removal of contaminated soils. Source mass removal subsequently should result in less contaminant mass leaching into groundwater, thereby, reducing and/or stabilizing groundwater concentrations at the site. Per our discussion, groundwater monitoring is also to be employed to ensure offsite receptors are not impacted by vapor intrusion. Plan for monitoring in the SMP is proposed as follows: Monitoring network; Analytical via EPA Method 8260 for Volatile Organic Compounds: Onsite - 3 monitoring wells (existing MW-1, proposed MW-4 and MW-5) o Install 2 additional monitoring wells in the assumed plume centerline 1 near/downgradient of the former Leavitts drycleaner sump 1 between the former Leavitts drycleaner and MW-1 Offsite - 2 wells (existing MW-2 and MW-3) Monitoring wells will be completed to a depth of 35 feet and up to 45 feet below ground surface using hollow stem auger to ensure the seasonally low water table is captured. Additional sampling may be coupled with the above effort as part of the Leaking Underground Storage Tank (LUST) closure regulated under the Division of Environmental Response and Remediation. 21st and 21st SMP Outline ■ Salt Lake City, UT February 17, 2022 ■ Terracon Project No. 61197460 2 Sampling Duration is proposed as: 5 years of groundwater monitoring o 1 year quarterly (4 sampling events) o 2 years semiannual (4 sampling events) o 2 years annual (2 sampling events) Plume Stability Criteria and Contingency: Once a stable and/or decreasing plume trend is established via an appropriate statistical method, groundwater monitoring for remedial action performance will be considered achieved. o Termination clause established for 8 data points early for statistical analysis o Post the establishment of a stable and/or decreasing plume, monitoring wells will be properly abandoned (if no longer in use by the LUST program) If plume stability is not reached within the proposed 5-year timeframe, Terracon will determine causation and perform appropriate actions, potentially including: additional investigation, additional remedial actions, and/or abatement. Offsite Vapor Intrusion monitoring and Contingency: Offsite monitoring wells (MW-2 and MW-3) will be additionally monitored for vapor intrusion o The SMP will detail Vapor Intrusion Screening Levels (VISL) applicable to the offsite monitoring through utilizing the EPA VISL Calculator based on land use. If offsite groundwater concentrations exceed the calculated threshold for appropriate land use this will trigger 1) soil gas sampling; if soil gas sampling exceeds thresholds, 2) sub slab soil gas sampling, to determine if the offsite plume is posing a vapor intrusion risk to offsite properties. If sub-slab sampling exceeds thresholds for vapor intrusion, 4 quarterly rounds of coupled sub slab and indoor air sampling will be conducted. If during any sampling event indoor air exceeds applicable thresholds and is verified by a coupled sub-slab point, emergency actions will be taken (i.e. mobile air purifiers). If consistent rounds of sampling demonstrate vapor intrusion occurring through coupled indoor air and sub slab sampling, abatement actions will be taken as appropriate (i.e. sub slab depressurization system installation). Multiple lines of evidence, a robust conceptual site model, building conditions review, data review and order of magnitude evaluations would be required for a full vapor intrusion investigation. These details are not scoped in the SMP since offsite monitoring wells are currently below the EPA RSL for vapor intrusion. It is not suspected that actions onsite will elevate groundwater concentration and initiate the above contingencies; they are included based on discussion. A general outline of steps is included in this memo for completeness of the receptor pathway accounting for all potential risk. Additional stimulations of the SMP: A VIMS design is under review by the DWMRC, and the construction completion of the VIMS will be included in the SMP. It is assumed the VIMS will be installed per the design; if the installation varies from the design, changes will be reviewed by the Professional Engineer and the DWMRC prior to modification to ensure the protectiveness level is not affected by the modifications. Environmental Deed Restrictions will be drafted and recorded as appropriate, in general they will: 21st and 21st SMP Outline ■ Salt Lake City, UT February 17, 2022 ■ Terracon Project No. 61197460 3 o Restrict groundwater use o Restrict soil excavation where waste is left in place at the property boundary o Restrict new development Vapor Intrusion clause for new development: an assessment of potential need for VIMS or other abetment actions for any new occupied spaces onsite. o Annual notification requirement stating these stipulations have been meet and maintained Including a checklist for the VIMS competency, slab competency, and other elements which might affect the integrity and protectiveness of the VIMS and/or vapor mitigation pathway If CR 10-4 or greater, and HQ of 1 or greater in onsite monitoring wells, Terracon must prove that the VIMS is protective for this risk range: Measure exhaust from pipes and sub-slab vapor (permanent points) If sub-slab exceeds applicable thresholds, perform sub-slab sampling coupled with indoor air sampling post a chemical inventory of the occupied space: o Analyze using EPA Method TO-15 for the “drycleaner” five PCE, TCE, cis-DCE, 1,1-DCE, and VC o Perform 4 quarters of sampling (first event includes installation) for sub-slab sampling and indoor air sampling o Clause for revision If land change use or modifications are necessitated in the future the SMP shall be revised as appropriate. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis.
