Dear Clint Dilley:The Division of Drinking Water (the Division) received your request for the Operating Permit for the rehabilitation of Haynes Well #2 Replace from Trevor Andra, District
Engineer for Magna, on July 9, 2024. Required chemistry data was submitted on July 23, 2024.
Our understanding of the project was the chemical and mechanical cleaning and rehabilitation of Haynes Well #2 Replace (identified as WS016 in the Division’s database). The scope of
work included mechanical cleaning, chemical treatment, performing a well video, screened zone redevelopment, constant rate test, installation of new pump and well disinfection. All chemicals
and materials used for rehabilitation and repair were specified to be ANSI/NSF 60 or ANSI NSF 61 certified.
We have received the following information for the Haynes Well #2 Replace Rehabilitation project:
Design engineer’s statement of conformance with approval conditions.
Evidence of O&M manual delivery.
As-built or record drawings.
Confirmation that the pump was replaced with the same size as the original and as such, the Drinking Water Source Protection Zones have not changed.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Water Quality Data- requested analyte data including antimony, beryllium, cyanide, thallium, aluminum, color and odor all demonstrating satisfactory results.
We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for the rehabilitation of Haynes Well #2 Replace (WS016)is hereby
issued as constituted by this letter. You may now place Haynes Well #2 Replace in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
