INSTRUCTIONS FOR A TEMPORARY PERMIT TO OPERATE AN
EXPLORATION AND PRODUCTION WASTE FACILITY
Step 1.Register the Company in DWMRC’s Community Portal.Complete Facility
Information.
The Community Portal is a database used by DWMRC and industry for accessing,submitting,
and tracking information for regulated Solid Waste facilities throughout the state of Utah.Each
Solid Waste facility regulated by DWMRC must be registered in the Community Portal.You will
use the Portal to submit quarterly and annual reports,request permit renewal or modification,
and pay Solid Waste fees.
To register,go to the following website:

1.1:Click Register.

1.2:Complete the information in the form and click Register

1.3:After you click Register,you will receive an email titled “Welcome to UT DWMRC Portal.”
The email will include a hyperlink.Click on the link and you will be taken to a website to change
your password.Enter your password in both boxes and click Change Password.
You will now be in the Community Portal.The home page is called the Portal Dashboard.

1.4:Enter your facility and contact information.Click the Facility tab.
●Click on “Add New Facility”

●Complete Facility Details and Click “Add”
●You have now added your facility and this step is complete.

Step 2.Complete the Temporary Permit Exploration and Production Waste Facility
Application Form.
Ensure all required information in the application is completed.

Step 3.Obtain the Following Required Documentation
a.Facility Map with each landfarm cell,landfill cell,and surface impoundment labeled.Indicate
any other site features.Example Below.
b.Copy of Division of Oil,Gas,and Mining Permit.
c.Copy of Division of Oil,Gas,and Mining Permit Approval/Issuance Letter.
d.Copy of Division of Oil,Gas,and Mining Boding (Financial Assurance)for facility.

Step 4.Submit Application Form and required documentation through the Community
Portal Miscellaneous Document Submission.
4.1:In the top,right hand corner of the Community Portal Home Page,click the tab labeled
“Miscellaneous Document Submission”.
In the “Choose a Program Field,select “Solid Waste”
4.2:Click the sub-program “Solid Waste.”
4.3:In the Comments field,enter “Temporary Permit Exploration and Production Waste Facility
Application,”your facility name,and the name of the person submitting the application.
4.4:Click “Upload Documents.”

4.5:In the pop-up box,click Upload Files.Select all file(s)containing:1)Completed Exploration
and Production Waste Facility Temporary Permit Application Form;2)Facility Map;3)Copy of
Division of Oil,Gas,and Mining Permit;4)Copy of Division of Oil,Gas,and Mining Permit
Approval/Issuance Letter;and 5)Copy of Division of Oil,Gas,and Mining Boding (Financial
Assurance)for facility.
After each file has been uploaded,click “Done.”
4.6:When all files have been uploaded,select “Submit.”
4.7:You will get an email stating that you have submitted documents to DWMRC.This step is
complete.
5.Within 120 days of DWMRC’s issuance of a temporary permit,the financial
assurance mechanism held by DOGM must be transferred to DWMRC.If the
Director issues a temporary permit,it shall not become effective until the
financial assurance mechanism held by DOGM is transferred to DWMRC.An
effective temporary permit is required to operate an E&P Facility under DWMRC’s
jurisdiction
Contact the financial institution or surety agent to complete this process.
