Rule: R309-105-10(4)(a), R309-515-8(1)(a)Spring collection areas shall be periodically (preferably annually) cleared of deep-
rooted vegetation to prevent root growth from clogging collection lines. This significant
deficiency must be corrected within 120 days of notification or have a corrective action
plan approved by DDW.
Notes:Roots seen in collection line at the bottom of the spring box. This was remediated after the
survey and will be reflected on an IPS Deficiency Correction Notice that will accompany the
submission of this Survey.

Girl's Home currently has an exception to this for the exact same Spring......Exception
