The incident location is depicted in the aftached site map (Appendix 1). lncident Background On December 4, 2023, at 1330 ET, A Zoley Trucking vehicle experienced a diesel fuel release. The release was reportedly due to damage to fuel tank and was contained to the asphalt roadway and adjacent soil shoulder. No drains or waterways were impacted by the release. Emergency Response On December 13,2023, at 1740 ET, Zoley Trucking retained ERTS to manage and oversee all regulatory and environmenlal reporting requirements. ERTS then dispatched Enviroserve of Salt Lake City, Utah to conduct all onsite cleanup operations. Regulatory Notifications On December 14,2023, at 1 101 ET, pursuant to the Utah Administrative Code, R315, Rules 315-9-1, and 315-9-4 ERTS notified the Utah DEQ regarding the release. Hannah Tremble- Marty took receipt of the call and report number 16817 was issued for the incident. Two separate areas were impacted and remediated by excavation. The southern area of excavation measured 13 feet by 9 feet, to a depth of 1 foot. The northern area of excavation measured '1 0 feet by 8 feet to a depth of 'l .5 feet. PID screenings and soil sampling were conducted at this time as well. Enviroserve backfilled the areas with clean soil and transported all impacted soil offsite for pending disposal. The analytical report is included as Appendix 2 and all contractor photos are included as Appendix 3. Waste Disposal On March 8, 2024, EnviroServe transported a total of 12.06 tons of diesel-impacted soil and debris to Wasatch Regional Landfill, Grantsville, Utah 84029 for disposal. Disposal documents are provided in Appendix 4. Conclusion and Recommendations The release has been remediated and no evidence was found that suggests any significant environmental impact remains on the site. ERTS suggests that this project be closed, and no further remedial action be taken.
