HomeMy WebLinkAboutDSHW-2024-007525July 30, 2024 Kathy Proctor, QRO Manager Amerisource Bergen Drug Corporation 1765 Fremont Drive Salt Lake City, UT 84104 RE:Close Out LetterCompliance Evaluation InspectionUTR000015917 Dear Ms. Proctor: On May 8, 2024 representatives of the Division of Waste Management and Radiation Control conducted a compliance evaluation inspection at your facility. The scope of the inspection was to determine compliance with the Utah Solid and Hazardous Waste Rules. The inspectors noted the following compliance issue: In accordance with R315-266-508(a)(2)(ii)of the Utah Administrative Code (UAC), “A healthcare facility shipping non-creditable hazardous waste pharmaceuticals shall write either the word “PHARMS” or PHRM” in Item 13 of EPA Form 8700-22. Amerisource Bergen had failed to mark their pharmaceutical waste with the word PHARMS or PHRM. Amerisource Bergen has registered as a small quantity generator until they can accurately prove to be a very small quantity generator. Amerisource Bergen has amended their hazardous waste generation spreadsheets to accurately reflect pharmaceutical waste and RCRA waste. As a result of this corrective actionyour facility is considered to be in compliance with the applicablerules. Please be aware that starting in 2021, all small quantity generators must re-notify using EPA form 8700-12 every four years in accordance with R315-262-18 of the Utah Administrative Code.
