(Terracon) is pleased to submit this Work Plan and Price to conduct four groundwater sampling events at the Former Blanding Chevron (Facility ID 5000247, Release ID GFB), located at 191 North Highway 191, Blanding, Utah. This Work Plan was prepared in response to the Work Assignment GFB-35-TER received by Terracon. This work will be conducted under Terracon's contract (Contract Number 216383) with the Division of Environmental Response and Remediation (DERR). OBJECTIVE Six underground storage tanks were removed from the site in June 1995. During excavation and tank removal, petroleum impacted soil and groundwater was discovered. DERR has been managing the site since the initial tank removal. Groundwater monitoring began in 1998 and indicated impacted groundwater present in both the shallow and deep aquifers on the site. Beginning in October 2006, light non-aqueous phase liquid (LNAPL) was reported in both shallow and deep monitoring wells. Vacuum recovery events were conducted at the site in 2008 to remove the LNAPL, and absorbent socks were installed in deeper monitoring wells. Periodic sampling of the remaining monitoring wells continued, with the most recent event in October 2019. The work plan price provides an estimate of costs for the implementation of the work plan, which includes quarterly groundwater monitoring, product removal, and reporting itemized per task. Work performed under each task or sub-task shall not commence without prior approval from the DERR. SCOPE OF WORK 2.1 Task 1a Groundwater Monitoring Event, August 2024 Terracon will attempt to collect groundwater samples from nine existing monitoring wells located at the site: Shallow wells MW-3B, MW-4B, and MW-8B, and deeper wells MW-3, MW-4, MW-5, MW-7, MW-8, and MW-10. Terracon will measure the free product thickness (if present) and depth to groundwater in each of the wells. Assuming free product is not present, Terracon will purge each well, using either new disposable bailers or a peristaltic pump with new tubing for each well. A Utah-certified Soil & Groundwater Sampler will collect a sample when three casing volumes have been purged from each well. Quality control samples consisting of a trip blank and a field blank (collected from the publicly accessible faucet spigot from the Blanding City public access well as per previous events) will be collected. This work plan assumes that purge water can be surface applied and allowed to reinfiltrate or evaporate. If free product is encountered, a petroleum absorbent sock will be placed in the well and will be replaced at the following groundwater monitoring event. Previous sampling events indicate that wells MW-3, MW-5, MW-7, and MW-3B had free product present. Disposal of the used socks is included in this Scope of Work. This work plan assumes that the petroleum absorbent socks can be dried out and disposed of as municipal waste. Up to nine groundwater samples and two quality control samples (field blank and trip blank) will be submitted to a Utah-certified analytical laboratory for analysis of methyl tertiary- butyl ether, benzene, toluene, ethylbenzene, total xylenes, and naphthalene (MBTEXN) using United States Environmental Protection Agency (EPA) Method 8260B, gasoline-range total petroleum hydrocarbons (TPH-GRO) using EPA Method 8260B, and diesel-range total petroleum hydrocarbons (TPH-DRO) using EPA Method 8015B. Petroleum absorbent socks will be removed from the wells that had free product in the prior monitoring event, if any, disposed of, and new petroleum absorbent socks placed in the wells. 2.4 Task 2b Groundwater Monitoring Report, December 2024 Task 2b includes preparing a Groundwater Monitoring Report following the Scope of Work outlined in Task 1b. 2.5 Task 1c Groundwater Monitoring Event, February 2025 Task 1c includes conducting a groundwater monitoring event at the site following the Scope of Work outlined in Task 1a. Petroleum absorbent socks will be removed from the wells that had free product in the prior monitoring event, if any, disposed of, and new petroleum absorbent socks placed in the wells. 2.6 Task 2c Groundwater Monitoring Report, March 2025 Task 2c includes preparing a Groundwater Monitoring Report following the Scope of Work outlined in Task 1b. 2.7 Task 1d Groundwater Monitoring Event, May 2025 Task 1d includes conducting a groundwater monitoring event at the site following the Scope of Work outlined in Task 1a. Petroleum absorbent socks will be removed from the wells that had free product in the prior monitoring event, if any, disposed of, and new petroleum absorbent socks placed in the wells. 2.8 Task 2d Groundwater Monitoring Report, June 2025 Task 2d includes preparing a Groundwater Monitoring Report following the Scope of Work outlined in Task 1b. Terracon makes no warranties, expressed or implied, regarding its services, findings, conclusions, or recommendations. Please note that Terracon does not warrant the work of laboratories, regulatory agencies, or other third parties supplying information used in the preparation of the report. These investigation services will be performed in accordance with the Scope of Work agreed with you, our client, as set forth in this proposal and are not intended to be in strict conformance with ASTM E1903-19. WORK PLAN BUDGET The proposed cost to perform this work is $8,174.90 per event for a total of $32,699.60. For budgetary purposes, Terracon has assumed that all the wells will be sampled. Petroleum absorbent socks have also been included in the budget. The actual costs of the monitoring events will be billed to the client. An itemized budget spreadsheet is attached, detailing the cost breakdown associated with each task. The cost estimate was generated using Terracon's 2023 PST Fund approved labor rates. If PST Fund Labor rates are changed, the Work Plan will be updated to reflect the new rates. If conditions are identified that will substantially alter the proposed Scope of Work, Terracon will notify DERR to discuss appropriate actions or modifications to this Work Plan. Terracon will not exceed the estimated costs without prior approval from the DERR Project Manager. ASSUMPTIONS The following assumptions were made during preparation of this Work Plan and Price: Terracon assumes that no site access issues will be encountered during the investigation. In the event Terracon encounters difficulty with access to any of the properties, DERR will be contacted immediately to determine appropriate courses of action. Terracon assumes that the monitoring wells proposed for sampling are in adequate working condition with sufficient water column to allow sampling and are not covered by asphalt or otherwise inaccessible. For cost estimating purposes, Terracon assumes 11 groundwater samples will be submitted to the analytical laboratory during each sampling event, one from each of the nine wells plus one field blank and one trip blank.
