Dear Mayor Noreen Johnson:The Division of Drinking Water (the Division) received the plans and specifications for equipping the Brinkerhoff Well and installing the well supply pipeline
from your consultant, Brennan Rees with Ensign Engineering, on July 9, 2024. The Division issued Plan Approval for drilling the Brinkerhoff Well on January 31, 2023. Written Review comments
were sent to your consultant on July 16, 2024, and revised plans and specifications were received on July 25, 2024.
Our understanding of the project is that the new Brinkerhoff Well (WS009) was drilled southwest of Bicknell Town and near the existing Brinkerhoff Spring (WS007). A 10-inch diameter
permanent steel casing along with steel louvered screen was installed to a depth of 600 feet. A 22-inch diameter surface casing was grouted in place to a depth of 110 feet and a steel
ring was welded between the production casing and the conductor casing. The well was test pumped for 24 hours at a constant rate of 1,200 gallons per minute (gpm) with a drawdown of
14.6 feet. The Brinkerhoff well will be equipped with a 100 HP submersible pump capable of producing 400 gpm. The well transmission pipeline will be a 10-inch diameter HDPE pipe that
will connect into the existing distribution system on the south side of Bicknell Town. Approximately 11,000 feet of 10-inch diameter well supply pipeline will be installed. This well
is identified as WS009 in the Division’s database.
We have received the following information for the Brinkerhoff Well and Supply Pipeline:
Certification of well seal.
Well driller’s report (well log).
Aquifer drawdown test results (step drawdown test and constant-rate test).
New Source Chemical Analysis of the well water.
Plans and specifications for equipping the well.
Pump information; 100 HP Submersible Pump & Motor, 400 gpm production rate
Wellhead discharge piping.
Pitless adapter unit and well house design
We have completed our review of the plans and specifications, stamped, and signed by Brennen R. Rees, P. E., and dated July 25, 2024,and found they basically comply with the applicable
portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for equipping the Brinkerhoff Well, wellhouse and well supply pipelineare
hereby approved.
This approval pertains to construction only. An Operating Permit must be obtained from the Director before the Brinkerhoff Well may be put into service. A checklist outlining the well
approval process, including the items required for issuing an operating permit for this well is enclosed for your information. Approvals or permits from the local authority or county
may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or
quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins
so that these inspections can be scheduled.
This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter. The project, as described herein,
is generally eligible for assistance under the Drinking Water Construction Assistance Program. All change orders will need to be reviewed and approved for compliance with the Drinking
Water Rules and the conditions of this construction approval. The project may now be advertised for bids. But, as a condition of the project funding, you must not award the contract
or proceed with construction until the Division of Drinking Water has authorized you to do so. This is necessary to comply with applicable requirements and to protect you, as the funding
recipient, by ensuring that the contract meets requirements for funded projects. Federal regulations require that a concerted effort must be made to encourage participation of small
and disadvantaged business enterprises (DBE) by providing requisite information, including plans and specifications on the project. In order to provide opportunities to DBE subcontractors,
utilization goals for Minority-owned Business Enterprises (MBE) and Women-owned Business Enterprises (WBE) have been determined to be 4% and 2%, respectively. As outlined in the contract
documents, the prime contractor must submit a list of prepared MBE/WBE subcontractors to be used on this project and documentation of solicitation efforts with the bid. The prime contractor's
efforts to provide opportunity to DBE's must be reviewed and approved by this office before the contract can be awarded.
