Shelley Executive Director DIVISION OF DRINKING WATER Nathan Lunstad Director January 24, 2024 Brett Jerold Knight Hyde Park City Water System 113 E Center Hyde Park, UT 84318 Subject: Notice of Source Deactivation for Hyde Park City Water System; UTAH03007 Brett Jerold Knight: This letter serves as notification that the Division of Drinking Water (the Division) has changed the activity status of the following sources from “Active” to “Inactive”:  100 EAST 350 N WELL (identified as WS002) The listed sources are now considered inactive and are not allowed to be utilized as a source of drinking water until they have been formally reactivated. In most cases, the reactivation process involves only collecting the outstanding samples that were missed during the source’s period of inactivity. The reactivation process does not require that systems obtain new operating permits. As of January 24, 2024, the following samples will need to be collected and reviewed by the Division prior to placing the source back online.
