Step 1: Fill out application completely. Incomplete applications are not accepted. Copy of most recent HAZWOPER certificate is required.
Step 2: Pay $275.00 recertification fee (exam only) OR $425.00 for Consultant Recertification fee and Consultant Renewal Course online at DERRpay.utah.gov and print receipt to attach to application.
Step 3: Email application and receipt to [e-mail] or mail to DERR, P.O. Box 144840, Salt Lake City, UT 84114-4840. Application and payment must be submitted 5 days prior to exam or course date.
Step 4: Register by email at [e-mail] and choose Consultant Renewal Course Date: _____________ OR choose from recertification exam dates provided by [e-mail]. Testing for the exam only is offered first Tuesday of each month at 9:00 AM or the third Tuesday of each month at 2:00 PM. The Consultant Renewal Course is offered in the spring and winter each year, dates are sent via email to consultants a month prior. Course replay is also an option that can be discussed.
