Director July 15, 2024 David Hewett Foothill Water Users Association 2268 North Cottontail Drive Cedar City, Utah 84721 Dear Mr. Hewett, Subject: RTCR Level 2 Assessment Foothill Water Users Association UTAH11099 Thank you for meeting with me on May 23, 2024, to conduct the Level 2 Assessment (L2) of your water system. Your cooperation in facilitating this assessment is greatly appreciated. Observations: The assessment was conducted due to four Level 1 assessments performed within a 12-month period. These assessments included routine, repeat, and Triggered Source Total Coliform Positive (TC+) and (EC+) Escherichia coli (E. coli) samples collected from December 2023 through May 2024. Foothill Water Users is a residential subdivision with 13 service connections. The system includes a well source, a 140,000-gallon above-ground steel storage tank, and a distribution system of unknown size and type. The distribution system also includes fire hydrants distributed throughout the area. The system is currently not approved due to multiple deficiencies in the well, tank, and distribution system. During the site visit, the steel storage tank was found to have multiple holes and seams coming apart, rendering it capable of holding only a few feet of water. The system is currently under a Boil Order as of the date of this letter and is in the process of replacing the tank and completing improvements to the wellhead building. Conclusions: 1. The TC+EC+ samples are likely due to outside contamination entering compromised storage tank. David Hewett Page 2 July 15, 2024 Deficiencies: Deficiencies were assigned on the Sanitary Survey (SS), as this was a combined SS/L2 site visit. Recommendations: 1. Distribution System O&M: Conduct disinfection and flushing of the distribution system in accordance with AWWA C651 standards. 2. Revise RTCR Sample Site Plan: Update the RTCR sample site plan to exclude sampling sites located at the ends of the system and submit the revised plan to the Division of Drinking Water (DDW). 3. Implement Unidirectional Flushing Program: Initiate a unidirectional flushing program with a goal of achieving a flushing velocity of 2.5 feet per second. For assistance, contact the Rural Water Association of Utah. 4. Follow Sanitary Sampling Protocol: Ensure that sampling locations are properly flushed and disinfected using a strong chlorine-based solution. This includes applying the solution, allowing adequate contact time, flushing the site, and collecting the sample.
