Dodge Soldier Summit SSD 2203 East Cedar Fort Drive Eagle Mountain, Utah 84005 Subject:Plan Approval, Water System Improvements (DS001); Soldier Summit SSD, System #25143, File #15452 Dear Brian Dodge:The Division of Drinking Water (the Division) received the plans and specifications for the proposed Water System Improvements from your consultant, Derek S. Anderson, P.E., and Sunrise Engineering on July 1, 2024. Written review comments were provided to your consultant on July 8, 2024. The Division received the revised plans and specifications on July 29, 2024. This project has met the conditions for receiving a plan approval. Our understanding of the project includes the following: The project includes the construction of approximately 10,700 lineal feet of new culinary water line with associated appurtenances (3,700 lineal feet of 10-inch diameter C900 PVC, 2,000 lineal feet of 10-inch diameter steel, and 5,000 lineal feet of 8-inch diameter C900 PVC) to replace old and undersized water lines. The project includes a new connection to an existing concrete water storage tank, a replacement water transmission line, installation of the water transmission line by jack and bore beneath UPRR and UDOT rights-of-way, and replacement of water distribution system piping on the north side of U.S. Hwy 6, including seven (7) water service connections. The required fire flow and duration for the project is 1,500 gallons per minute (gpm) for 2 hours. The facility included in this project is identified as DS001 in the Division’s database. We have completed our review of the plans and specifications, stamped, and signed byDerek S. Anderson, P.E., dated June 21, 2024,and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for Water System Improvements (DS001) are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before Water System Improvements (DS001) may be put into service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter. If you have any questions regarding this approval, please contact Chris Martin, P.E., of this office, at (385) 271-7040, or me at (385) 515-1464. Sincerely, Michael Newberry, P.E.
