Dear Paul Swainston:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Cub River 1 MG Tank from your consultant, Marcus Simons, P.E. on
July 16, 2024. The Division issued Plan Approval for this project on August 24, 2022. Division staff conducted an onsite final inspection of this project on July 12, 2024.
Our understanding of the project was the construction of an above-ground 1 MG concrete storage
tank located just north of the Cub River Sports Complex near 200 East 11600 North in Lewiston,
Utah. Water is supplied to this tank via the new Sports Complex Production Well (WS005, File
#12566).
We have received the following information for Cub River 1 MG Tank, (ST003):
Certification of Rule Conformance with plan approval conditions by a professional engineer.
Record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Evidence of O&M manual delivery.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Cub River 1 MG Tank, (ST003)is hereby issued as constituted
by this letter. You may now place Cub River 1 MG Tank, (ST003) in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
