RE: Operating Permit Request for Purgatory Booster Pump Station

Dear Paul;

The Washington County Water Conservancy District (WCWCD) has requested that we submit
this operating permit request letter to you for the issuance of an operating permit for File #13508
– Purgatory Booster Pump Station (PF003). In accordance with R309-500-9 we certify the
following:

1) No significant changes were made to the plans for the booster pump station.
2) Conformance with the plan approval conditions.
3) O&M Manuals were given to the owner.
4) As-built drawings are provided to the Owner and DEQ with this letter.
5) Proper flushing and disinfection were accomplished according to ANSI/AWWA C654-97
standards and Bac-T results are included with this letter.

The District would like to introduce the booster pump station into the Quail Lake to Sand Hollow
Regional Pipeline system as soon as possible.
