Dear Zachary Renstrom:The Division of Drinking Water (the Division) received your request for the Operating Permit for the Sand Hollow Well 15 (WS024) from Todd Gardner, Alpha Engineering
on July 17, 2024. Previously, the Division received the plans and specifications for the well house and equipping on June 25, 2021. Those plans also included Sand Hollow Well 11 (WS021)
and Well 13 (WS023) well house designs as they all were using the same design. Well equipping plan approval for those two wells was issued on October 13, 2021. To go along with the previously
submitted well house design, well pump specifications along with a design report that included the new source chemistry results, pump test results, well driller report and the well seal
certification for Sand Hollow Well 15 were submitted to the Division on December 1, 2022. The Division issued Plan Approval for drilling Well 15 (WS024) on February 25, 2021, and Plan
Approval for equipping the well on December 19, 2022.Our understanding of the Sand Hollow Well 15 (WS024) includes a vertical turbine line shaft pump with a 585 gallon per minute (gpm)
capacity at 638 ft of total dynamic head. However, it will generally be operated at about 460 gpm. It has a 125-horsepower motor with a variable frequency drive to be able to provide
varied flow as needed. The well was pump tested at approximately 650 gallons per minute at which drawdown stabilized at 516 feet of drawdown. This project also included a new well house
with all required discharge valving.
We have received the following information for Sand Hollow Well 15 (WS024):
Design engineer’s statement of conformance with approval conditions.
Documentation of valid water right.
Evidence of O&M manual delivery.
As-built or record drawings.
Documentation that the requirements for coverage under the Washington County source protection ordinance have been met.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Additional water quality data that had been requested in the well equipping approval letter dated December 19, 2022.
We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for Sand Hollow Well 15 WS024) is hereby issued as constituted by
this letter. You may now place Sand Hollow Well 15 (WS024) in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
The equipped well pump capacity of Sand Hollow Well 15 is 585 gpm but will be generally operated at 460 gpm. The safe yield of Sand Hollow Well 15 is rated at 433 gpm, which is calculated
based on two-thirds of the constant-rate aquifer drawdown test results at 650 gpm. The safe yield of 433 gpm is the basis for determining the maximum number of connections that Sand
Hollow Well 15 can serve.
Drinking Water Source Protection Requirement
The Division received proof that a map of the source protection zones was provided to Hurricane City on March 8, 2024. The Division concurred with the final Drinking Water Source Protection
(DWSP) plan in a letter dated March 28, 2024. The next updated DWSP Plan is due by December 21, 2028. If you have questions about the source protection requirements, please contact Deidre

Issuance of this Operating Permit changes your water system’s monitoring and reporting
requirements. The monitoring requirements for this well are given below. An updated monitoring schedule can be viewed any time at waterlink.utah.gov under the Water Monitoring section.

is the Division's understanding that Sand Hollow Well 15 (WS024) will either be treated at the Arsenic & Manganese Treatment Plant (identified as TP001 in the Division’s inventory) or
blended with additional Sand Hollow Wells and treatment plant effluent (identified as ‘Arsenic Blend for Sand Hollow Regional Pipeline (SHRP) and Sand Hollow (SH) Wells’ - TP020).Compliance
with the arsenic standard under these scenarios is determined through quarterly sampling at the Arsenic & Manganese Treatment Plant (TP001) and the blended location (TP020). Please continue
to collect all quarterly samples on waterlink.utah.gov and submit quarterly arsenic treatment and blending reports to the Division. Please contact David Kruse at (385)-566-7789 or dbkruse@utah.gov
