Everett Director ERRL-0284-24 July 3, 2024 CERTIFIED MAIL RETURN RECEIPT REQUESTED Jason Miller Springville City 110 South Main Street Springville, Utah 84663 Re: Additional Sampling Request and Requirements Whitehead Power Facility, located at 777 North 425 West, Springville, Utah Facility Identification No. 1000990, Release OGA Dear Mr. Miller: The Division of Environmental Response and Remediation (DERR) has reviewed the documents for this Release Site, including a Subsurface Investigation Report (2023) and a line removal Closure Notice (2023) provided by you or your consultant. After reviewing this information, the DERR project manager recommends further investigation before a determination of no further action (closure) can be considered. The Division Director requires that five grab soil samples be collected from surface soils along the waterline level of the pond embankments that were impacted by the 2022 petroleum release at the power plant. Two of the samples should be collected nearest to where the petroleum sheen was daylighting into the first surface pond. The other three samples should be taken from the farthest areas of the pond system where the sheen may have collected due to water flow or surface water cleanup activities (booms or other absorbent materials used to collect sheen or limit its movement). All environmental samples must be collected by a Utah-certified groundwater and soil sampler following sampling protocols outlined in Utah Admin. Code R311-205. The report and map showing the sample locations associated with these requirements must be submitted to our office within 90 days of your receipt of this letter. Environmental samples must be analyzed for diesel using the methods indicated in the enclosed Table of Analytical Methods for Sampling. NOTE: The sample preparation method and any modification(s) to a certified method must be reported by the laboratory.
