Meadows:The Division of Drinking Water (the Division) received the plans and specifications for the proposed Storage Tank Refurbishment of Reservoir #1 (ST002) and the Culinary Water System Improvements (DS001) from your consultant, Jonathan Johansen, P.E., with Johansen & Tuttle Engineering on May 26, 2024, and May 19, 2024, respectively. These projects have met the conditions for receiving plan approval. Storage Tank Refurbishment, File #14797 Our understanding of the Storage Tank Refurbishment is the refurbishment of the water storage tank identified as Reservoir #1 (ST002). The scope of work includes cleaning metal surfaces and applying paints and protective coatings. The tank will be cleaned and disinfected prior to being placed back into service. Culinary Water System Improvements, File #14626 Our understanding of the Culinary Water System Improvements is the installation of ten service connections, 1,120 lineal feet of 8-inch diameter PVC C900 pipe, 2 fire hydrant connections, and two (2) 8-inch gate valves with boxes and lids. We have completed our review of the plans and specifications, stamped, and signed byJonathan W. Johansen, P.E., dated May 17, 2024, and find they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for Storage Tank Refurbishment, Reservoir #1 (ST002) and Culinary Water System Improvements, Distribution System (DS001) are hereby approved. This approval pertains to construction only. An Operating Permit must be obtained from the Director before Storage Tank, Reservoir #1 (ST002) and Culinary Water System Improvements may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information. Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.The project, as described herein, is generally eligible for assistance under the federal Drinking Water State Revolving Fund (DWSRF) Financial Assistance Program. All change orders must be reviewed and approved for compliance with the Drinking Water Rules, the conditions of this construction approval, and the DWSRF Program’s financial assistance requirements.The project may now be advertised for bids. But, as a condition of the project funding, you must not award the contract or proceed with construction until the Division of Drinking Water has authorized you to do so. This is necessary to comply with applicable requirements and to protect you, as the funding recipient, by ensuring that the contract meets requirements for funded projects. Federal regulations require that a concerted effort must be made to encourage participation of small and disadvantaged business enterprises (DBE) by providing requisite information, including plans and specifications on the project. In order to provide opportunities to DBE subcontractors, utilization goals for Minority-owned Business Enterprises (MBE) and Women-owned Business Enterprises (WBE) have been determined to be 4% and 2%, respectively. As outlined in the contract documents, the prime contractor must submit a list of prepared MBE/WBE subcontractors to be used on this project and documentation of solicitation efforts with the bid. The prime contractor's efforts to provide opportunity to DBE's must be reviewed and approved by this office before the contract can be awarded.This project is also required to comply with provisions outlined in the Davis-Bacon Wage Act, American Iron and Steel requirements, and the Build America, Buy America Act.
