Leigh Brian Head Town Water System PO Box 190068 Brian Head, Utah 84719 Subject:Plan Approval, Well Drilling, Salt Pile Well (WS011); Brian Head Town Water System, System #11001, File #14461, SRF #3F1910 Dear Chris Leigh:The Division of Drinking Water (the Division) received the plans and specifications for the proposed drilling of the Salt Pile Well from Todd Gardner, Alpha Engineering, on May 8, 2024. A Preliminary Evaluation Report (PER) had not been submitted at that time. It was later submitted to the Division and after a series of reviews and comments, Division staff could concur with the PER on July 3, 2024. We understand that this project consists of drilling a minimum 28-inch diameter surface borehole to a depth of 120 feet. A 24-inch diameter permanent conductor casing will also be installed to a depth of 120 feet. The well will be grouted between the borehole and surface casing to a depth of 120 feet. Salt Pile Well will be drilled to a total depth of approximately 800 feet with a 22-inch diameter well borehole and a 16-inch diameter production steel casing, 240 feet long, and HSLA 0.70” louvered shutter screen at various depths totaling another 560 feet.The surface casing (24-inch) and the blank steel casings (16-inch) will be ASTM A53 Grade B black steel. Gravel will be placed between the permanent production casing and the borehole from 100 feet below Ground Surface (BGS) to 800 BGS and between the permanent conductor casing and the production casing from the ground surface to 100 ft BGS. A steel ring will be welded at the top of the casing between the 24-inch surface casing and the 16-inch production casing to seal the inner annular space where the gravel pack will be installed. A 2-inch telemetry tube will be installed in the gravel pack from the surface to the bottom of the well. There will also be a 6-inch gravel refill port welded at the surface of the annular ring and will be capped. The project also includes well development, logging, and pump tests. The Salt Pile Well is identified as WS011 in the Division’s database. We have received the following information for the Brian Head Salt Pile Well drilling project: Plans and specifications for borehole and well drilling. Preliminary Evaluation Report. We have completed our review of the plans and specifications, stamped, and signed by Todd Brent Gardner, P.E., and dated July 3, 2024, and found they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems. On this basis, the plans for drilling the Salt Pile Well are hereby approved. This approval pertains to well drilling, development, aquifer testing, and disinfection of Salt Pile Well only. Please be aware that discharge permits may be required by Utah Division of Water Quality for discharges generated during well drilling and aquifer drawdown testing. The Utah Division of Water Rights (State Engineer’s Office) regulates the drilling of water wells. Before the drilling of a well commences, the well driller must receive a start card from the State Engineer’s Office. Please be aware that, for public drinking water supply wells, in addition to the Division of Drinking Water’s R309-515 rule, the Division of Water Rights’ rule R655-4 also applies and shall be followed. After drilling is completed, you are required to submit additional information outlined in R309-515-6(5)(b) and (c) for review andobtain an approval from the Director for equipping this well and constructing discharge piping and infrastructure necessary for introducing the well water into the distribution system. After obtaining the well equipping approval, you’ll then be required to obtain an Operating Permit before Salt Pile Well may be put in service. A checklist outlining the well approval process, including the items required for well equipping and an Operating Permit, is enclosed for your information. Please label the well water sample collected for new source chemical analysis with your water system number UTAH11001 and WS011 (for both the facility ID and sample point ID) on all laboratory forms for each individual source. This will ensure proper identification and entry of the new source chemical analysis results in our database. Approvals or permits from the local authority or county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial materials have not been ordered within one year of the date of this letter. Concurrence – Preliminary Evaluation Report The Division received the Preliminary Evaluation Report (PER) for the Salt Pile Well from your consultant, Todd Gardner and Alpha Engineering on May 27, 2024, and additional information on June 24, 2024 and July 2, 2024. The Division concurs with this report. The PER must be refined, and a complete Drinking Water Source Protection (DWSP) Plan is submitted within one (1) year of the date of this letter. Refer to R309-600-13(6) and R309-600-7(1). The administration and enforcement of the Iron County ordinance is the responsibility and obligation of the owner of the Public Water System (refer to Section 13.16.080). If the Iron County ordinance is used in lieu of land use agreements, the administrative contact for the system will need to provide a signed and dated statement indicating the following: (PWS NAME) hereby acknowledges that Iron County Code Title 13.16: Drinking Water Source Protection will be used to protect source protection zones one and two of the (SOURCE). Given that protection under the Iron County Code is the responsibility of the Public Water System (PWS), (PWS NAME) commits to the following: The PWS will monitor the parcels located within source protection zones one and two. The PWS will inform any landowner of the source protection delineation and county source protection ordinance should any development occur on their property. The PWS will inform Iron County and the Southwest Utah Health Department should any property development appear to violate Chapter 13.16 of the Iron County Code. SRF Funding The project, as described herein, is generally eligible for assistance under the Drinking Water Construction Assistance Program. All change orders will need to be reviewed and approved for compliance with the Drinking Water Rules and the conditions of this construction approval. The project may now be advertised for bids. But, as a condition of the project funding, you must not award the contract or proceed with construction until the Division of Drinking Water has authorized you to do so. This is necessary to comply with applicable requirements and to protect you, as the funding recipient, by ensuring that the contract meets requirements for funded projects. Federal regulations require that a concerted effort must be made to encourage participation of small and disadvantaged business enterprises (DBE) by providing requisite information, including plans and specifications on the project. In order to provide opportunities to DBE subcontractors, utilization goals for Minority-owned Business Enterprises (MBE) and Women-owned Business Enterprises (WBE) have been determined to be 4% and 2%, respectively. As outlined in the contract documents, the prime contractor must submit a list of prepared MBE/WBE subcontractors to be used on this project and documentation of solicitation efforts with the bid. The prime contractor's efforts to provide opportunities to DBE's must be reviewed and approved by this office before the contract can be awarded.
