Temple St, RM 1205 NATURAL RESOURCE SERVICES Salt Lake City, Utah 84150-6320 Dear Mr. Beecher, Subject: RTCR Level 2 Assessment for Indianola LDS Chapel Utah20073 Thank you for meeting with me on May 11, 2024, to conduct the Level 2 Assessment (L2) of your water system. Your cooperation in facilitating this assessment is greatly appreciated. Observations: This assessment was triggered by two Level 1 assessments conducted within a 12-month period, including routine Total Coliform Positive (TC+) samples in September 2023 and February 2024. The repeat samples for these assessments were either not taken or were delayed, with September 2023 repeats not completed and February 2024 repeats taken in March 2024. Additionally, a leak was discovered at the pit-less adapter on the wellhead in April 2024. Conclusions: 1. The leak at the wellhead is likely the cause of the TC+ sample results. However, the Level 2 Assessment was also triggered by the failure to obtain repeat samples in the same month as the TC+ routine sample. Repeat samples must be collected in the same month as the TC+ routine sample. Recommendations: 1. Adhere to RTCR and Ground Water Rule Requirements: Ensure compliance with RTCR and Ground Water Rule regulations regarding repeat sampling and triggered source sampling following a TC+ routine sample. Contact the Division of Drinking Water (DDW) if clarification on follow-up sampling procedures is needed. 2. Schedule Routine Samples Appropriately: Schedule routine samples for the first day of the month, the first day of the week, and avoid holidays when laboratories may be closed. Andrew Beecher Page 2 July 11, 2024 3. Follow Proper Sanitary Sampling Protocol: Ensure that sampling locations are properly flushed and disinfected using a strong chlorine-based solution. This includes applying the solution, allowing adequate contact time, flushing the site, and then collecting the sample. 4.
