Notes:There was a gasket, but it was not deteriorated to the point it needs replacing

inches below any opening and shielded to prevent the entrance of contaminants. This
significant deficiency must be corrected within 120 days of notification or have a
corrective action plan approved by DDW.
Notes:The air vent had been hit by a snow-cat during the ski season and is no longer verticle
enough to maintain a 90 degree angle.

Notes:There is a lock on the tank lid, but the hinges do not attache to the access box. The hinges
need to be secured to the access hatch so that the lock can lock be useful.

Rule: R309-545-6(1) and 545-9 Rule requires openings in a storage tank roof or top, designed to accommodate control
apparatus or pump columns, to be welded, gasketed, or curbed and sleeved and to have
additional proper shielding to prevent vandalism. This significant deficiency must be
corrected within 120 days of notification or have a corrective action plan approved by
DDW.
Notes:There is an electrical conduit into the access hatch that needs some sealant around it to
prevent contamination in the tank.
