Access opening lid properly gasketed Rule: R309-545-14(2)Rule requires the access opening to a tank to be furnished with a gasket between the lid and frame. This significant deficiency must be corrected within 120 days of notification or have a corrective action plan approved by DDW. Access opening locked Rule: R309-545-14(3)Rule requires the lid to any access opening to have a locking device. This significant deficiency must be corrected within 120 days of notification or have a corrective action plan approved by DDW. Notes:The tank requires a new lock. Deficiency Points: 25 Days until points become effective on IPS: 120 SDWIS Deficiency Description: V029 STORAGE TANK ACCESS OPENING LACKS A LOCK Sanitary Survey Category: FW SDWIS Severity Code: Significant Deficiency Storage Tanks | ST002 | PIONEER TANK 79A. Vent is either down-turned or shielded from contaminants (at least 2 inches below the bottom of the opening) Rule: R309-545-15(1)Rule requires inverted vents on water storage tanks to be down-turned a minimum of 2 inches below any opening and shielded to prevent the entrance of contaminants. This significant deficiency must be corrected within 120 days of notification or have a corrective action plan approved by DDW. Notes:The air vent had been hit by a snow-cat during the ski season and is no longer verticle enough to maintain a 90 degree angle. Access opening locked Rule: R309-545-14(3)Rule requires the lid to any access opening to have a locking device. This significant deficiency must be corrected within 120 days of notification or have a corrective action plan approved by DDW. Notes:There is a lock on the tank lid, but the hinges do not attache to the access box. The hinges need to be secured to the access hatch so that the lock can lock be useful. Roof or wall penetrations sealed Rule: R309-545-6(1) and 545-9 Rule requires openings in a storage tank roof or top, designed to accommodate control apparatus or pump columns, to be welded, gasketed, or curbed and sleeved and to have additional proper shielding to prevent vandalism. This significant deficiency must be corrected within 120 days of notification or have a corrective action plan approved by DDW. Notes:There is an electrical conduit into the access hatch that needs some sealant around it to prevent contamination in the tank. Deficiency Points: 100 Days until points become effective on IPS: 120 SDWIS Deficiency Description: V017 STORAGE TANK SUBJECT TO CONTAMINATION DUE TO UNSEALED OPENINGS ON TANK ROOF OR SIDEWALLS Sanitary Survey Category: FW SDWIS Severity Code: Significant Deficiency Storage Tanks | ST004 | WEBSTER SMALL TANK 108C. Access opening lid properly gasketed Rule: R309-545-14(2)Rule requires the access opening to a tank to be furnished with a gasket between the lid and frame. Notes:This is the one where if the outer casing surrounding the bolted access hatch has a gasket, it could be considered compliant. If the pump station is used to pressurize the distribution system, does it have at least two functioning pumping units? ... if yes, answer the following question(s) Rule: R309-540-5(4)(b)Rule states inline booster pumps, withdrawing water directly from distribution lines without storage and feeding water directly into distribution lines, shall have at least two pumping units.
