In accordance with the Resource Conservation and Recovery Act, and the requirements ofthe Utah Hazardous Waste
Management Rules, I am submitting our Certification of Electronic Filing Form for the 2005 Biennial Hazardous Waste
Report.
Following the directions provided from the Division of Solid and Hazardous Waste, I used the Easitrak online software
program to produce oiu: biennial report. Once all of the data had been entered I followed the directions to submit the report
and the web site indicated that the report had been transferred to the appropriate state office.
The enclosed form was filled in with applicable data and certified as required by the letter and instructions sent to us. If you
need further information or have questions about the report please contact me.
