In accordance with the Resource Conservation and Recovery Act, and the requirements of the Utah Hazardous Waste
Management Rules, I am submitting our Certification of Electronic Filing Form for the 2007 Biennial Hazardous Waste
Report.
Following the directions provided from the Division of Solid and Hazardous Waste, I used the Easitrak online software
program to produce our biennial report. Once all of the data had been entered I followed the directions to submit the report
and the web site indicated that the report had been transfened to the appropriate state office.
The enclosed form was certified as required by the Declaration Form and the instructions sent to us. If you need further
information or have questions about the report please contact me.
