Management Rules, I am submitting our 2011 Biennial Hazardous Waste Report.
Following the directions provided from the Division of Solid and Hazardous Waste website (hitp://^vM'w.hazardous-wasie.uiah. eov/) I
downloaded the Florida Biennial Report Software to complete the 2011 Biennial Report, as instructed requested on the state's
website. I have included a print off of the forms generated from this software, with certifying official signature, and a disc with
the electronic data output from this software.
