Dear Levi Jones:The Division of Drinking Water (the Division) received a request for an Operating Permit for the proposed Spring #2 Improvements from your consultant, Matt Filla P.E.,
JUB Engineers, on June 28, 2024. The Division issued Plan Approval for this project on May 31, 2023. Division staff conducted an onsite final inspection of this project on June 10, 2024.
This project included the replacement of a spring collection box for Spring #2 (WS002), the replacement of approximately 300 feet of existing asbestos pipe with 8-inch diameter DR-25
C900 PVC water pipe, the installation of a new 8-inch Octave Ultrasonic Flowmeter inside a new concrete vault and related work. A new fence and berm were also installed around the spring
collection area.
We have received the following information for Spring #2 Improvements, Spring #2 (WS002):
Certification of Rule Conformance with plan approval conditions by a professional engineer in conformance with rules R309-500 through 550
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Evidence of O&M manual delivery.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Spring #2 Improvements, Spring #2 (WS002) is hereby issued
as constituted by this letter. You may now place Spring #2 (WS002) in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
