Corrective action documentation:

1. Distribution, 40 Water mains susceptible to nearby contamination sources
Action: state park staff initially added an additional vacuum breaker, however the final
resolution was to remove one hose to a separate spigot. Both are now on individual spigots with
individual vacuum breakers.

2. Distribution, 41B. Air relief valve not downturned
Corrective action: State Parks Regional Crew replaced the valve with a downturned end

3. Distribution, 41B Air relief valve not downturned
a. This seems to be a repeat of number 2?

4. Storage Tanks, 93C Gasket needs replacing on both lids
a. State Parks Regional Crew replaced both gaskets with heavy duty rubber.

5. Storage Tanks, 99B End of storage tank overflow pipe lacks No. 4 screen
a. State Parks Regional Crew added No. 4 screen to all 4 overflow pipes
