Box 96 Bicknell, Utah 84715 Subject:Operating Permit, 300,000 Gallon Storage Tank #2 (ST002); Bicknell Town, System #28001, File #12616, SRF #3F1786 Dear Mayor Noreen Johnson:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Storage Tank #2 (ST002) from your consultant, Kelly Chappell with Ensign Engineering on November 15, 2023. However, due to the contractor needing to clean, flush and disinfect the storage tank again after that date, the operating permit was put on hold. The Division issued Plan Approval for this project on April 28, 2022. The DEQ Central District Engineer conducted a final inspection of this facility on August 3, 2023, and again on July 1, 2024. Our understanding of the project is that a new 300,000-gallon concrete storage tank has been constructed adjacent to the existing Storage Tank #1. The new tank was constructed at the same elevation as the existing tank and both tanks receive chlorinated water from the mountain springs as well as the Brinkerhoff Spring. The new 300,000-gallon concrete tank is identified as Storage Tank #2 (ST002) in the Division’s database. We have received the following information for Storage Tank #2 (ST002): Certification of Rule Conformance with plan approval conditions by a professional engineer As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for Storage Tank #2is hereby issued as constituted by this letter. You may now place Storage Tank #2in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
