As you are aware representatives of the Utah Solid and Hazardous Waste Control Board (the Control Board) inspected TF009 prior to its shipment to UTTR and were present during disposal. In order to complete our inspection, you are hereby requested to submit the following information on or before October 15, 1993: 1. Please provide a copy of the hazardous waste manifest which accompanied the TF009 segment to the UTTR facility. 2. Please indicate when TF009 became the property of the U.S. government. Specifically, when was the paperwork signed transferring ownership from Hercules to the U.S. government. 3. The Hazardous Waste Disposal Plan for TF009 indicates that flexible linear shape charges are going to be used after the burn to "cut up" the carbon fiber casing and that the casing will be disposed of at an appropriate industrial waste facility. Please provide copies of all manifest or shipping papers used for the disposal of the TF009 casing. Also, indicate how Hercules established that the casing could be disposed of as solid waste at an industrial waste facility. 4. Also, if available, please provide photographs of TF009 before unloading, after unloading, and after disposal. Printed on recycled paper Given the nature of this disposal operation, we understand that some portions of this request may be confidential, should Hercules choose to request confidentiality, a confidentiality claim pursuant to R315-11 of the Utah Administrative Code will be necessary. If you have questions, please contact Bill Wallner at 538-6107. Sincerely, Dennis R.
