Re: Exception Report
1 1 1991
Dear Mr. Downs,
This letter is to inform you that Manifest 11003 from Chemical
Waste Management's OSCO facility was not received within the 45 day
requirement.
Personnel at the OSCO facility were called on
February 19, 1991 which was the 35th day from shipment, and
informed that Hercules, Inc. had not received a signed copy of
Manifest No. 11003. They replied that because of ice build-up in
some of the drums that they were not able to process the material.
OSCO was contacted again on March 1, 1991 which was the 45th
day from shipment to find out the progress of the material. They
replied that the drums still contained ice and they were not able
to close out the second page of the Manifest, and that they would
inform Hercules as soon as the waste was processed. They stated
that page 1 of 2 had been mailed on February 25, 1991. Hercules
received page 1 of 2 on March 4, 1991.
A copy of the generator's copy is enclosed.
Sincerely,
E. Richard Anderson, Manager
Environmental Planning
and Operations
