This letter is to inform you of manifests that were received from
Chemical Waste Management Inc.'s SCA incinerator past the 45 day
requirement. The manifests were for lab packs that Chemical Waste
Management's Technical Services Division packed and shipped from the Bacchus
Works. The manifests were faxed to the Bacchus Works on February 8, 1990
which was the fiftieth (50) day from the date of shipment.
The manifests that were late are:

- Illinois 4128090
- Illinois 4128091
- Illinois 4128092
- Illinois 4128093
- Illinois 4128094
Chuck Reilly of Chemical Waste Management's Technical Services Division
was contacted by phone on January 24, 1990 and was infotnned that manifests
for the lab pack shipment had not been received by Hercules and that it was
the thirty-fifth day from shipment.
A letter was faxed to Chuck Reilly on January 31, 1990 informing him that
the manifests still had not been received and that it was the forty-second
day from shipment. Stephanie Rhine of Chemical Waste Management's Technical
Service Division was also contacted by telephone the same day informing her
of the problem with the manifests.
Chemical Waste Management's SCA incinerator was called by Janet Bass of
my staff on Febiruary 7, 1990 and informed them that the manifests had not
been received at Bacchus Works and that it was now forty nine days from the
date of shipment. Janet asked that the manifests be faxed to Bacchus Works.
