SUBJECT: Meyerhoffer Well Sampling
On the morning of April 5, 1990, Don Verbica and Ed Costomiris of the Bureau of Solid
and Hazardous Waste met Mr. Meyerhoffer property owner, Melvin Meir of Salt Lake
County, and Shannon Storevd and Shane Hirschi both of Hercules Inc. The purpose of
the meeting was to obtain water samples from the private well behind Mr. Meyerhoffer's
home located at 6910 West 3800 South Magna, Utah.
Prior to arriving on-site, 40 ml glass samples vials were brought to the State Health
Laboratory, and a field blank sample was prepared. The blank, sample number HW9006,
consisted of two air free 40 ml vials of deionized water. The label on the vials were
filled out, the containers tagged and put in a sealable plastic bag. A chain of custody
seal was put on the plastic bag. The sample was then placed on ice.
Purging of the meyerhoffer well began at 0935 and terminated at 0950. A sample was
obtained by both the Bureau of Solid and Hazardous Waste and Hercules representatives.
Each sample consisted of two 40 ml glass vials. BSHW samples were prepared identically
as the blank, described above. The sample ID number is HW9007.
The samples will be analyzed for volatile organic compounds. The sample obtained by
BSHW will be analyzed in the State Health Laboratories. The sample obtained by the
Hercules representative will be analyzed by Enseco Laboratory of Denver, Colorado.
The samples were delivered to the State Health Laboratory by BSHW personnel and the
chain of custody maintained by laboratory staff; a copy of the chain of custody record
is attached.
