Dear Mr. Anderson,
ATK Launch Systems, Promontory has prepared this report to summarize activities
conducted under Emergency Permit No. UT-031-2017 between October 1, 2017 and
December 29, 2017. All de-mining activities authorized by this pennit were performed
on the Kosmo test range. De-mining activities successfully treated the flares as verified
through visual inspection, and the remaining residue/debris was properly disposed in the
ATK Promontory Landfill. Activities conducted under this permit are outlined below:

Annual Disposition Update
As requested by the Division, the following is an update on the current disposition of
flares that have not been located or have been marked for recovery but not yet retrieved
On January 10, 2017, ATK Launch Systems provided an update for the disposition of
flares that were marked but not retrieved. Eight flares still remained on the test range
despite recovery efforts. A flare reported missing in June 2015 has now been added to

this list. No additional progress was made in locating the missing flare during 2017. A
summary of those flares with no change in disposition is shown in Table 1.
Table 2 summarizes flares not located or retrieved since the 2017 report but with known
locations and a good probability of recovery. On October 31,2017 a recovery attempt
was made to excavate flares with known locations that had buried themselves on impact.
Flares listed in Table 2 have a known location, but could not be recovered due to wet soil
conditions at the site. Conditions will be monitored and a recovery attempt will be
planned for 2018.
