Anderson Division of Waste Management and Radiation Control Department of Environmental Quality PO Box 144880 Salt Lake City, UT 84114-4880 APR I 4 2017 D5HW-20I7-0032M RE: Report for Emergency Permit #UT-039-201^ ATK Launch Systems - Promontory Facility UTD009081357 Dear Mr. Anderson, ATK Launch Systems, Promontory has prepared this report to summarize activities conducted under Emergency Permit No. UT-039-201&between January 1, 2017 and March 31,2017. All de-mining activities authorized by this permit were performed on the Kosmo test range. The de-mining activity conducted on January 16, 2017 partially treated a dropped LUU 19 flare. Visual inspection of the flare after de-mining revealed the illuminant had been consumed, but the explosive bolts did not appear to initiate. The recovery team packaged the remaining component and transferred to Promontory for final thermal treatment at the M-136 bum grounds. On Febmary 4, 2017 the component was treated in a clam-shell container with donor material to ensure the small amount of explosive in the bolts initiated. The remaining residue/debris was properly disposed in the ATK Promontory Landfill.
