Box 707 Brigham City, UT 84302-0707 RE: Well H-4 Colloidal Borescope Investigation Report ATK Launch Systems – Promontory Facility UTD009081357 Dear Mr. Gooch: The Division of Waste Management and Radiation Control has completed its review of the Borescope Investigation Report (April, 2016). ATK and Geotech Environmental Equipment conducted the borescope investigation at well H-4 and submitted the report in response to the Division’s concern regarding the extent of groundwater contamination and flow direction in the area. This issue was discussed as a finding in the 2014 Operation and Maintenance (O&M) Inspection. The report indicates that a preferential pathway for groundwater flow direction in well H-4 was not identified. Therefore, it appears that flow and plume migration to the southwest along lake sediments is still possible. ATK sampled piezometer BC-3, which is located south, southwest of well H-4 in the Fall of 2015 and the Spring of 2016 without detecting perchlorate. However, perchlorate was detected at 10 ug/L in BC- 3 in the Spring of 2015. If groundwater in the area does flow southwest, another well exists that is in a fairly good location to detect a plume migrating from well H-4. This well is shown on the Lampo Junction Quadrangle Map, USGS 7.5 Minute Series and is located at Township 11 North, Range 6 West in the Southeast ¼ of the Northeast ¼ of Section 1 at an elevation of 4,300 feet. It is approximately two miles southwest of well H-4. The owner and potential uses of the well are unknown. After discussing this well with my staff, Paul Hancock has agreed to look into the ownership of the well and to try and obtain access to determine if the well is viable for collecting groundwater samples. If the (Over) owner will allow access and it is determined the well can provide representative groundwater samples, the Division would like to have the well sampled in the Spring of 2017. If this can be achieved, the Division believes the ongoing monitoring of the well along with BC-3 may allow the extent of groundwater contamination in the area southwest of well H-4 to be adequately defined.
