Dear Mr. Anderson,
ATK Launch Systems, Promontory has prepared this report to summarize activities
conducted under Emergency Permit No. UT-012-2015 between July 1,2015 and
September 28,2015. All de-mining activities authorized by this permit were performed
on the Kosmo test range. De-mining activities successfully treated the flares as verified
through visual inspection, and the remaining residue/debris was properly disposed in the
ATK Promontory Landfill. Activities conducted under this permit are outlined below:

During testing in August, fourteen M257 flares failed to properly deploy. Six flares
buried themselves under the soil surface upon impact. However, the GPS locations of the
entry points were lost. A team has been assembled to resurvey the area and determine if
the entry points can be located from physical markers. The search is expected to occur
between October 13th and 14th. Orbital ATK will provide an update to the Division at the
conclusion of this search. Additionally, the precise demining time and location of the

other eight flares was lost. A general area has been provided based on wind conditions
during the tests. The demining time is estimated from the test schedule.
