Dear Sir or Madame:
The above-referenced policy ("Policy") was issued on behalf of AllianWAbCystems Inc. ("Insured") by Great
American E&S Insurance Company ("Company") as evidence of theitGOTcial assurance required by Utah law
for Closure or Post-Closure Care of the above-referenced facilitie^tne Policy expired on June 28. 2015.
Prior to expiration, the Company offered renewal terms to the Mfiurea and also provided the Insured with an
option to extend the Policy until September 1, 2015, for an a^wohal premium. In response, the Insured
elected not to pay the additional premium for the extensiafOmdid it pay the premium for the quoted renewal.
As such, the Policy was non-renewed for non-paymentaffpremium.
In light of the elections made by the Insured and itswure to pay the required premium, the Policy expired and
the Company's obligation to provide financial assurance for the above-referenced facilities ended effective
June28. 2015. /V>
