Anderson 2D\ 3-0012S2- Division of Solid and Hazardous Waste Department of Environmental Quality PO Box 144880 Salt Lake City, UT 84114-4880 RE: Report for Emergency Permit #UT-031-2012 ATK Launch Systems - Promontory Facility UTD009081357 Dear Mr. Anderson, ATK Launch Systems, Promontory has prepared this report to summarize activities conducted under Emergency Permit No. UT-031-2012 between October 1, 2012 and December 29, 2012. All de-mining activities authorized by this permit were performed on the KOSMO test range. De-mining activities successfully treated the flares as verified through visual inspection, and the remaining residue/debris was properly disposed in the ATK Promontory Landfill. Currently, efforts to locate the flare for demining have been unsuccessful but are ongoing. The flare is believed to be within the boundaries of the KOSMO test range.
