Anderson Division of Solid and Hazardous Waste Department of Environmental Quality PO Box 144880 Salt Lake City, UT 84114-4880 RE: Report for Emergency Permit #UT-020-2012 ATK Launch Systems - Promontory Facility UTD009081357 Dear Mr. Anderson, ATK Launch Systems, Promontory has prepared this report to summarize activities conducted under Emergency Permit No. UT-020-2012 between July 2, 2012 and September 30, 2012. All de-mining activities authorized by this permit were performed on the KOSMO test range. De-mining activities successfully treated the flares as verified through visual inspection, and the remaining residue/debris was properly disposed in the ATK Promontory Landfill. Activities conducted under this permit are outlined below: Date Type # Flares De-mining Engineer Location Time July 3, 2012 LUU2 Robert Goodell +41° 41'00.6" -112° 48' 26.5" LUU2 Robert Goodell +41° 41* 2.1" -112° 48' 24.9" LUU2 Robert Goodell +41° 40'55.4" -112° 48' 29.8" 100 130 1045 September 27, 2012 LUU2 Dave Merrill +41° 41' 53.1" -112° 47 36.9" 1220 As communicated in the July 9, 2012 report, eleven M278 flares failed to ignite during testing on June 29, 2012, and were buried in mud upon impact with the ground. The entry points were marked for each flare, but no excavation could be performed due to unstable soil conditions. Soil conditions did not improve sufficiently during evaluations in August and September to attempt excavation. The next attempt to recover buried flares will occur during the week of October 22, 2012. Additionally, as reported by e-mail to the Division on February 14, 2012, a LUU2 flare failed to deploy during testing on February 10, 2012. Currently, efforts to locate the flare for demining have been unsuccessful, but continue during subsequent testing/cleanup efforts. Ociober i!. 2012 Mr Scott I.
