RE: Report for Emergency Permit #UT-007-2012
ATK Launch Systems - Promontory Facility UTD009081357
Dear Mr. Anderson,
ATK Launch Systems, Promontory has prepared this report to summarize activities
conducted under Emergency Permit No. UT-007-2012 between April 2, 2012 and June
30, 2012. One day of testing, June 29, 2012, occurred during the permitted period.
During this testing, eleven flares failed to ignite and were buried in mud upon impact
with the ground. The entry points were marked for each flare, but no excavation could be
performed due to unstable soil conditions. Therefore, no de-mining activities occurred
during the permitted period. Attempts will be made to recover the buried flares once soil
conditions become suitable for excavation.
Additionally, as reported by e-mail to the Division on February 14, 2012, a LUU2 flare
failed to deploy during testing on February 10, 2012. Currently, efforts to locate the flare
for demining have been unsuccessful, but are ongoing.
If you have any questions, comments or need additional information, please contact Jason
Wells at 435-863-6895.
Sincerely,
Robert Ingersoll
