As required by the requirements in 40 CFR 264.143 and 40 CFR 264.144, ATK Launch
Systems is providing signed duplicate original certificates of insurance for closure and
post-closure care for our Bacchus and Promontory facilities. The revised cost estimates
for closure and post-closure care were itemized in our June 5, 2012 correspondence. If

under my direction or supervision in accordance with a system designed to assure that
qualified personnel properly gather and evaluate the information submitted. Based on

warrants that such policy conforms in all respects with the requirements of R315-8-8 [which incorporate 40 CFR
264.143(e), 264.145(e)], or R315-7-15 [which incorporate 265.143(e), and 265.145(d)] as applicable and as such
regulations were constituted on the date shown immediately below. It is agreed that any provision ofthe policy
inconsistent with such regulations is hereby amended to eliminate such inconsistency.
Whenever requested by the Executive Secretary ofthe Solid and Hazardous Waste Control Board ofthe State of
